Objective:The purpose of this study was to analyze the ultrasonographic features of various uterine diseases,investigate the law of the ultrasonic changes and provide image basis of transvaginal color ultrasound in diagnosis of uterine diseases for clinical gynecology doctors.Methods:All data were collected from hospitalized patients with uterine diseases diagnosed by transvaginal ultrasound and treated in the First Hospital of Jilin University from January 1st 2016 to December 31th2016.All patients underwent surgery or biopsy,and finally confirmed by pathology.960 cases of uterine cavity lesions,1011 cases of uterine body lesions and 349 cases of cervical lesions were inrolled.A respective analysis was made about the outcomes of these 2320 cases' transvaginal ultrasound.I made a detailed ultrasonic feature of uterine diseases,and compared the sensitivity,specificity,positive rate and negative rate of transvaginal ultrasound in the diagnosis of uterine diseases..Results:1.Transvaginal ultrasonography and surgery confirmed a variety of uterine diseases in 2320 cases,which was consisted of.960 cases of uterine cavity lesions,verified by final postoperative pathology,ultrasonic correct detection rate of 92.2%,the misdiagnosis rate was8.7%,the missed diagnosis rate was 8.7%,sensitivity was 94.3%,specificity of 91.3%,positive predictive value was 86.5% and negative predictive value of 96.5% and the Kappa value of ultrasound for diagnosis of uterine lesions was 0.840;1011 cases of uterine body lesions,uterine lesions involving the detection rate of ultrasonic diagnosis was98.4%,the misdiagnosis rate was 1.4%,the missed diagnosis rate was1.9%,sensitivity 98.1%,specificity of 98.6%,positive predictive value98.3%,negative predictive value was 98.3%,the Kappa value of ultrasonic diagnosis of uterine body was 0.967;349 cases of cervical lesions,the right of ultrasonic diagnosis of cervical lesions was 85.3%,sensitivity was 80.0%,the misdiagnosis rate was 14.7%,the missed diagnosis rate was 20.0%,specificity of 100%,positive predictive value was 100%,negative predictive value of 95.7%.2.The main ultrasonic images of endometrial lesions mainly include 156 cases of Heterogeneous endometrial echo and 804 cases of abnormal endometrial echo.Differences of US color flow between the benign and the malignant groups showed statistical significance(P <0.05).Most malignant intrauterine lesions appeared signs of blood flow,even rich blood flow signs.3.Differences of echopatterns of intrauterine lesions exist statistical significance(P < 0.05).Most endometrial polyps showed hypoechoicchanges,as well as endometrial malignancies.The main manifestation of pregnancy related diseases is the change of mixed light group echo.However,the submucosal myoma of uterus is mainly low echo.4.The thickness of endometrium was changed because of different pathological changes.The intimal thickness of malignant lesions was greater than that of benign lesions,which is statistical significance.5.1008 Cases of uterine benign lesions in uterine morphology,muscle wall echo,boundary and the number of lesions were significantly different.However,there was no difference in the echo types,blood flow distribution and diameter.6.There were 229 benign lesions and 120 malignant lesions in the total 349 cases of cervical lesions.149 cases of cervical polyp and 80 cases of cervical myoma were included in the 229 cases of benign lesons.The statistical analysis showed that there were significant differences in echo types in different cervical lesions(p < 0.05).The main manifestations of cervical polyps are hyperechoic changes,and cervical fibroids and cervical cancer are mainly manifested as low echo changes,however,there was significant difference between the two groups in the blood flow.Cervical cancer is mainly characterized by abundant blood flow signals,while cervical fibroids are not.Conclusions:1.Doppler ultrasonography is of high value in the diagnosis ofuterine cavity lesions and uterine body lesions,however,cervical lesions need to be combined with internal examination or cervical screening.In order to avoid missed diagnosis of some diseases.2.Blood flow signal and endometrial thickness can differentiate endometrial benign and malignant lesions.Most malignant lesions showed blood flow signals,especially abundant blood flow signals,however,it should be combined with other clinical data to exclude the possibility of pregnancy related diseases?The endometrial thickness of malignant lesions was higher than that of benign lesions.3.Uterine morphology,muscle wall echo and boundary conditions can be used as the identification of uterine fibroids and adenomyosis.The uterine morphology of most uterine leiomyoma is abnormal,and the myometrial echo is even,little difference is between the single lesion and multiple lesions.The uterine morphology of adenomyosis is plump and spherical,and the myometrial echo is uneven,the proportion of single lesion was higher.4.The main manifestations of cervical polyps are hyperechoic changes,and cervical fibroids and cervical cancer are mainly manifested as low echo changes,however,cervical cancer is mainly characterized by abundant blood flow signals,which could distinguish the cervical fibroids and cervical cancer.
