Efficacy Of Tripterygium Hypoglaucum(Levl.) Hutch Mixture In Blood-heat Type Of Psoriasis Vulgaris And Its Effect On IL-12 And IL-23

Objective: To observe the efficacy and safety of tripterygium hypoglaucum(levl.)hutch mixture in blood-heat syndrome of psoriasis vulgaris.Investigate the mechanism of drug action from microcosmic angle.Provide the basis for clinical application of Tripterygium hypoglaucum(levl.)hutch mixture in psoriasis vulgaris.Medthods: Using methods of randomized controlled trial,divided 66 patients of blood-heat syndrome of psoriasis vulgaris into the experimental group and control group.There were 33 cases in each group.Recorded their data in detail and tested the IL-12 and IL-23 by collecting blood samples from them.Meanwhile,tested the IL-12 and IL-23 from 30 healthy volunteers which were selected for comparison.The experimental group was given Tripterygium hypoglaucum(levl.)hutch mixture.The control group was given Acitretin Capsule.Both of them were given Paeonol Ointment.Every patient would return visit once a week.After continuous therapy for 8 weeks,reexamined the IL-12 and IL-23,then judged the efficacy.Analyzed the data by using SPSS 22.0.Results: 1.Among 66 patients,there were 1 case rejected from the experimental group and 3 cases rejected from the control group.62 valid cases were collected.Research objects from two groups had comparability in baseline data such as age,sex,course,disease,IL-12 level and IL-23 level(P>0.05).2.After treatment,the TCM syndrome evaluation of the experimental group and the control group were both significantly decreased(P<0.05).The experimental group was better than the control group(P<0.05).3.After treatment,the PASI score of the experimental group and the control group were both significantly decreased(P < 0.05).There was no significant difference between two groups(P>0.05).4.Before treatment,the IL-12 and IL-23 level of the experimental group and the control group were both significantly higher than healthy volunteers(P<0.05).After treatment,both of them were significantly decreased(P<0.05).When compared their IL-12 level and the IL-23 level of the experimental group with healthy volunteers,there was no significant difference between groups(P>0.05).But the IL-23 level of the control group was still higher than healthy volunteers(P<0.05).5.After 7 days of treatment,the total effective rate of the experimental group was higher than the control group(P<0.05).When treatment continued 14 days,21 days and 28 days,the total effective rate of them had no significant difference(P>0.05).After treatment,the total effective rate of the experimental group was 90.63% and the cured and markedly effective rate was 71.88%.The total effective rate of the control group was 83.33% and the cured and markedly effective rate was 60.33%.The experimental group was higher than the control group,but there was no significant difference between two groups(P>0.05).6.There was no severe adverse reaction in two groups.There were 3 cases in the experimental group who had mild abdominal pain and abdominal distension.These adverse reactions relieved by reducing times of taking medicine.There were 6 cases in the control group who had xerosis cutis and oral mucosa dryness.With no treatment,these adverse reactions relieved with the lapse of administration time.There were 3 cases in the control group who had transaminase slightly elevated.After drug withdrawal for 2 months,their transaminase level recovered to normal.7.Within 3 months after treatment,there were 4 cases in the experimental group and 9 cases in the control group who had psoriasis vulgaris recurrence.There was a significant difference between two groups(P<0.05).Conclusion: Tripterygium hypoglaucum(levl.)hutch mixture has a good efficacy in blood-heat syndrome of psoriasis vulgaris.There is no obvious adverse reaction.Compare with Acitretin Capsule,tripterygium hypoglaucum(levl.)hutch mixture has advantages such as shorter onset time,lower recurrence rate,milder adverse reaction and so on.
