Risk Factors Analysis Of Children Refractory Pneumonia In A Mono-center In Urumqi

Objective:In order to explore the risk factors and clinical characteristics of refractory pneumonia in children.To provide a strong basis for the early identification and treatment of refractory pneumonia in children.Improve the prognosis of refractory pneumonia.Methods:Using the clinical epidemiological method.219 children with refractory pneumonia in the first affiliated hospital of Xinjiang medical university from December 2011 to December 2016 were selected as case group,400 patients with common pneumonia were randomly selected as control group.The clinical data were analyzed retrospectively.First,the risk factors of refractory pneumonia may be analyzed by single factor analysis,followed by the significant single factor logistic regression analysis.The observation items included 8 clinical and 17 test indexes and imaging diagnosis data.Results: Age,anemia,rickets,Nutrition,congenital heart disease,congenital heart disease,airway dysplasia of congenital,multi-resistant infection,atopic physical,WBC,N(serum),LDH,CRP,ESR,PCT,IL-6.25(OH)D,Ig M.Ig G,IgA,CD3+,CD4+,CD8+,serum complement C3,C4 were closely related to refractory pneumonia(P<0.05).Multivariate conditional logistic regression results:Whether the congenital heart disease(OR = 3.908),Merge dysplasia(OR = 3.718),Infiltration area(OR = 11.422),LDH(OR = 3.288),Pleural effusion(OR = 11.66),Lobar lung consolidation and atelectasis(OR = 6.57),malnutrition(OR = 3.036),age(OR = 1.764),ESR(OR =1.263),were independent risk factors for refractory pneumonia,P<0.05.Conclusions:The results of single factor analysis show that all the indicators except pneumonia mycoplasma infection in this study were the risk factors of refractory pneumonia in children.Among them,age,congenital heart disease,airway dysplasia of congenital,malnutrition,CRP,ESR,LDH,large consolidation shadows are independent risk factors(P<0.05),which can help early diagnosis,early treatment,slow complications and improve prognosis.
