Objectives:To observe the clinical efficacy and safety of safflower yellow combined with low molecular weight heparin in the prevention of deep venous thrombosis after total knee arthroplasty,and to explore its mechanism.Methods:From November 2015 to June 2016,60 patients who meet the inclusion criteria of primary unilateral total knee arthroplasty were gathered from the thirddepartment of bone and joint surgery of Wendeng orthopaedic hospital.According to the admission order number,the patients were randomly divided into group A,group B and group C.The group A was given 5000IU of low molecular weight heparin per day 12 hours after operation.Group B was given safflower yellow 150mg per day after operation,and low molecular weight heparin 5000IU was administrated per day meantime.Group C was given safflower yellow 150mg per day after operation,and low molecular weight heparin 2500IU was administrated per day meantime.D-Dimer value,prothrombin time,activated partial thromboplastin time,C-reactive protein value,serum interleukin-6 values,VAS scores,volume of drainage,platelet count,hemoglobin,thrombosis and adverse reaction were compared before operation and 1,3,14 days post operation among three groups.All the datas were analyzed by SPSS20.0.Results:60 cases were included in this study.After random completion,there are 20 cases in each group.During the treatment,one patient in the group A took aspirin on his own,while one patient was discharged in advance in group C.There were significant differences of group A and group B,C in D-Dimer value,prothrombin time,activated partial thromboplastin time,C-reactive protein value,serum interleukin-6 values,VAS scores(p<0.05)while there were no significant differences between group B and group C(p>0.05)at the same time point on the 1st,3rd and 14th day after operation.There was no statistically significant difference in volume of drainage between the three groups.The incidence of DVT in group A was 5.26%,while group B and C were 0%and there was no statistically significant difference.There were significant differences of group A,B and group C in the PLT and HGB(p<0.05)while there were no significant differences between group A and group B(p>0.05)at the same time point on the 1st,3rd and 14th day after operation.There was no significant differences in three groups of ecchymosis area the first day after operation(p>0.05).There were significant differences of ecchymosis area in group A,B and group C(p<0.05)while there were no significant differences between group A and group B(p>0.05)on the 3rd and 14th day after operation..Conclusions:Safflower yellow combined with low molecular weight heparin could effectively ameliorate the postoperative hypercoagulable state,reduce postoperative pain and bleeding,and could effectively decrease IL-6 and CRP levels.The clinical effect is satisfactory,and the side effect is small in the prevention of deep venous thrombosis of lower limb,and it is worthy of clinical promotion.
