Background & Objective: Chronic tonsillitis(CT,chronic tonsillitis)often caused by recurrent acute tonsil inflammation or tonsils socket drainage not free,bacteria and viruses in crypt caused infection.Tonsil hypertrophy(TH,tonsillar hypertrophy),is a kind of physiologic or pathologic hypertrophy phenomenon occured in tonsil tissue,repeatedly inflammation stimulation,pharyngeal infection,malnutrition,acute infectious diseases and physical quality are the causative factors [3],especially in the cold,damp and changebal climate areas.Childhood acute infectious diseases,malnutrition and physical factors may also induce tonsil hypertrophy.This assay investigated the expression difference of IL-4(interleukin-4)?TNF-?(tumor necrosis factor-?)?Foxp3(forkhead box P3)and CTLA-4(Cytotoxic T Lymphocyte Antigen 4)in both TH group and CT group and discussed their immunologic functions and the biological mechanisms in the development of chronic tonsillitis.Methods: 35 patients who suffered from chronic tonsillitis(CT)and 23 patients suffered from tonsillar hypert rophy(TH)were studied.We used HE staining to observe the pathological changes of the two groups.We used immunohistochemical experiment to detect the expression level and the distribution characteristics of IL-4,TNF-? and Foxp3 in the two groups.We also used immunofluorescence technique to detect CTLA-4 expression differences in the two groups.We used Real-time fluorescent quantitative PCR experiments to test the expression of IL-4m RNA,TNF-?m RNA,Foxp3 m RNA and CTLA-4m RNA in tonsil tissues of two groups.Results: HE staining showed that the main characteristics of pathogeny structure in CT group tonsil tissue were the changes of follicular hyperplasia and germinal center,while TH group had obvious "star" phenomenon with follicular hyperplasia.Immunohistochemical results found that relative to the TH group,the expression of IL-4,TNF-? and Foxp3 in CT group were significantly increased and the difference was statistically significant.Immunofluorescence results showed that the average fluorescence intensity and density of CTLA-4 protein in CT group was significantly stronger than the TH group,the difference was statistically significant.The q RT-PCR experiment results showed that the expression level of IL-4m RNA,TNF-?m RNA,Foxp3 m RNA and CTLA-4m RNA in CT group were all obviously higher than the HT group(n=58,t=6.294,P value were P<0.01,P<0.05,P<0.0,5,P<0.01 respectively),the differences were statistically significant too.Conclusion: IL-4,TNF-?,Foxp3 and CTLA-4 played different and important roles in the immune response in CT disease respectively and affected the development of the inflammatory response in the CT disease.
