Background: Lung cancer can be divided into non-small cell lung cancer and small cell lung cancer.Small cell lung cancer(SCLC)is about 25% of the total number of lung cancer.Its main characteristics is a high degree of malignancy,clinically easy to transfer,and the high proportion of early metastasis and a wide range of metastatic trend,poor surgical treatment,but more sensitive to chemotherapy.At present,the main chemotherapy regimen for small cell lung cancer is cisplatin or carboplatin combined with etoposide for the treatment of the industry recognized standard treatment mode.But because of its high degree of malignancy,the proliferation rate is too fast,easy to early metastasis.And many patients have been found in advanced lung cancer which lost the best treatment period.These patients have poor prognosis and low survival rates.Therefore there is a need for a clinical screening,monitoring efficacy,prognosis of biomarkers.Clinical widely used detection indicators are mainly NES,CEA,CYFRA21-1,CA125,Pro-GRP and so on.Although the clinical diagnosis of early diagnosis of lung cancer done a lot of research work,early positive diagnosis rate is still low.In recent years,scientists and related scholars have found a new type of tumor markers: thymidine kinase 1(TK1).TK1 and cell proliferation rate related to the diagnosis and treatment of nausea tumors have a wide range of effects.TK1 has a very important clinical valueObjective: To evaluate the value of serum TK1 in the early diagnosis and chemotherapy of small cell lung cancer.Methods: From January 2014 to June 2016,57 patients with small cell lung cancer were enrolled in the Department of Elderly Respiratory Medicine,the First Affiliated Hospital of Anhui Medical University.There were 39 cases of smokers,18 cases of non-smoking patients,45 cases of male patients and 12 cases of female patients.The median age was 62 years between 41 and 71 years.In this study,37 patients in the extensive period and 20 cases of limited period of patients.Extensive period often refers to the spread of lesions beyond the above areas while limited period defined as a radiation field can be surrounded by lesions,often confined to the side of the chest and local lymph nodes.All patients were clinically diagnosed with small cell lung cancer,taking etoposide + cisplatin standard treatment program,and all patients do not merge other malignancies.Clinically,at least 4 cycles or more platinum-like treatment regimen,can not tolerate cisplatin patients to be applied carboplatin.All patients were given at least 4 cycles or more of platinum-based treatment.Patients who can not tolerate cisplatin are treated with carboplatin.At the same time,57 cases of hospitalized patients were collected as control group,all benign lung disease,no malignant tumor.These patients included 22 males and 35 females,aged between 17 and 75 years old.All patients were immunochemically enhanced chemiluminescence method to determine the concentration of thymidine kinase 1 in serum and the positive results were determined as TK1> 2 pmol / L.Results: TK1 detection of small cell lung cancer under the ROC curve area of 0.837.Its sensitivity is higher than NSE.The expression of TK1 was correlated with the clinical stage of small cell lung cancer(P = 0.019)and the postoperative effect(P = 0.000).The expression of TK1 was not significantly correlated with sex,age and smoking(P> 0.05).The level of TK1 in small cell lung cancer before treatment was significantly higher than that in control group(P <0.05).The level of TK1 was gradually decreased in patients with 2 cycles of chemotherapy and 4 cycles,and the difference was statistically significant(P <0.05).Conclusion: TK1 can be used as early diagnosis and chemotherapy evaluation index of small cell lung cancer patients.And it has important reference value in guiding clinical stage and prognosis judgment.
