ObjectiveThe purpose of this study was to investigate the therapeutic effect and possible mechanism of auricular acupressure therapy on gastrointestinal reactions induced by chemotherapy in patients with gynecological malignancies.To seek a safe,economical and effective treatment plan to improve the quality of life of patients with malignant tumor.MethodsThis study was a prospective single-blind,randomized controlled trial.105 patients who met the inclusion criteria.were randomly divided into the following groups:auricular acupressure group,western medicine group and blank control group(n=three)in each group(n=35).The patients in the auricular acupressure group were given auricular acupressure therapy in the day before chemotherapy and second days after chemotherapy.The patients in the western medicine group before chemotherapy to 30 min were intravenously injected Ondanstron.Patients in the blank control group received only basic treatment without any preventive antiemetic measures.Time window:fourth days after chemotherapy.Observation index:the toxic and side effects of chemotherapy,the grade of gastrointestinal symptoms,the level of nausea and vomiting,the improvement of appetite,and the level of serum 5-HT before and after chemotherapy.All data were analyzed with SPSS19.0 software package.P<0.05 was found to be statistically significant.Results①Digestive tract symptom score:three groups of patients in the chemotherapy day,the first day after chemotherapy and chemotherapy for the first second days,the digestive tract symptom score difference was statistically significant(P<0.05).The symptoms of gastrointestinal symptoms were 2 days after chemotherapy.Auricular acupressure group and control group differences in the first day of chemotherapy and chemotherapy in second days were statistically significant(P<0.05),there was no significant difference with the western medicine group(P>0.05),suggesting that auricular acupressure could significantly improve gastrointestinal symptoms,compared with the efficacy of Western medicine group were no statistically difference.②Nausea and vomiting:three groups of patients with nausea and vomiting occurred more frequently in 24h after chemotherapy.The auricular acupressure group relief rate was 61.77%,compared with the blank group(36.35%),the difference was statistically significant(P<0.05).In western medicine group,the relief rate was 77.14%,which was better than that of auricular acupressure group,but the difference between the two groups was not statistically significant(P>0.05).The auricular acupressure group vomiting remission rate was 76.47%,compared with the control group(51.56%),the difference was statistically significant(P<0.05),and could reduce the number of additional Oberoi during chemotherapy.In western medicine group,the remission rate was 82.58%,which was better than that of auricular acupressure group,but there was no significant difference between the two groups(P>0.05).③The degree of appetite improvement:the first day of chemotherapy and the second day of chemotherapy in the auricular acupressure group compared with the blank group could improve appetite,the difference was statistically significant(P<0.05).There was significant difference between the western medicine group and the blank group in the second days of chemotherapy(P<0.05).④ There was no significant difference between the three groups in constipation and diarrhea(P>0.05).The results showed that the selected auricular points had no obvious effect on improving the symptoms of constipation and diarrhea.⑤Three groups of patients before and after chemotherapy KPS score,biochemical indicators,the difference was not statistically significant(P>0.05),indicating auricular point sticking safe and feasible.⑥The serum 5-HT value in three groups after second days of serum 5-HT values were significantly increased than before chemotherapy,the serum 5-HT was no statistically significant difference between the value of chemotherapy in patients with three groups(P>0.05),before and after comparison,there was significant difference(P<0.01).The serum 5-HT value of nausea and vomiting in the blank group was higher than that of 5-HT before and after chemotherapy,and the difference was statistically significant(P<0.05).The serum 5-HT value of nausea and vomiting in the auricular acupressure group was higher than that of the 5-HT before and after chemotherapy,but the difference was not statistically significant(P value was greater than 0.05),The results showed that one of the mechanisms of preventing and treating nausea and vomiting associated with chemotherapy in the treatment of auricular acupressure may be related to the decrease of serum 5-HT value.Conclusion:①The side effects of chemotherapy were more than 1-2 days after chemotherapy and acute nausea and vomiting was the highest incidence.②The curative effect of auricular acupressure therapy to alleviate chemotherapy related nausea and vomiting was clear,had certain curative effect on acute nausea and vomiting,significantly delayed nausea and vomiting effect,and could reduce the number of additional Oberoi during chemotherapy,compared with the effect of 5-HT receptor antagonist,a little difference,but the difference was not statistically significant.③ Auricular acupressure could improve the appetite of patients,especially in the first day of chemotherapy and chemotherapy for second days.④auricular acupressure therapy was safe.⑤Auricular acupressure could reduce the serum 5-HT,which may be one of the mechanisms of the prevention and treatment of chemotherapy-induced nausea and vomiting...
