| ObjectiveTo discuss the articulation characteristics and speech rehabilitation training methods of postoperative patients of ankyloglossia with articulation disorders,and offer evidences for clinical speech evaluation and speech therapy for these patients.Methods176 patients with articulation disorders with a history of lengthening surgery of the tongue-tie were selected as research objectives.They all came from speech therapeutic room in the First Affiliated Hospital of Zhengzhou University during July 2015 to January 2017,male 131 cases,female 45 cases,aging from 4 to 29,The ratio of male to female was 2.91:1,median age is(7.93±5.30)years old.The articulation characteristics of 176 patients were given speech evaluation by using the computer speech lab produced USA KAYPENTAX company according to speech intelligibility test table.70 patients who intelligence and hearing is normal,aging from 4 to 8,these patients met the inclusion criteria were recorded in according to the Chinese speech intelligibility test table respectively before and after treatments,and the effect of the treatment was evaluationed according to the materials before and after speech training.Statistical analysis was performed by SPSS17.0 software.The type of experimental design of this research was selfpaired design and paired t-test was performed for the speech training effect,α=0.05.Results 1.The articulation characteristics mainly shows replacement and omission,and lateralization takes the second place.2.From the pronunciation position,the percentage of occurrence of dysphonia after the operation is as follows:blade-palatal(93.8%)、supradental(82.45%)、backlingual consonant(68.2%)、 blade-alveolar(63.6%)、 lingua-palatal phoneme(61.4%)、labiodental(27.3%)and bilabial(22.7%).3.The articulation disorders of /b/、/d/、/t/、/q/、/x/、/z/、/c/were replaced by /j/ when these consonants were combined with /i/;The lingua-palatal phoneme、supradental and blade-palatal were lateralization when these consonants were combined with /i/;The blade-alveolar of /l/ articulation error percentage is higher than the backlingual consonant;/p/、/t/、/q/mainly showed unaspiration and was replaced by /b/、/d/、/j/.4.The error percentage is relatively low and the error form is simple when 21 consonants were combined with /a/、/o/、/e/,but error percentage is high and the error form is complex when 21 consonants were combined with /i/、/u/、/ü/.5.The study also found that the consonants were omission when /m/、/d/、/n/、/l/、/g/、/h/、/s/、/r/ combined with /u/ or when /n/、/l/、/j/、/q/ combined with /ü/;The articulation disorders of /g/、/z/、/s/、/zh/were replaced by /d/ or /k/、/c/、/ch/ were replaced by /t/ when these consonants were combined with /u/;lingua-palatal phoneme pronunciation errors mainly showed replacement in the same position of articulation.6.After treatment,the phonetic intelligibility of 70 patients has increased from 41.34 before treatment to 83.00 after treatment,incresing by 42.03,the differences were statistically significant(t=-29.254,P﹤0.001).ConclusionThe articulation characteristics mainly shows replacement and omission and lateralization takes the second place.Effective scientific treatment methods should be established based on the characteristics of postoperative patients with dysarthria.The training methods this research established are significantly effective,which can greatly improve the patients’ speech intelligibility. |
