| ObjectiveTo analyse the articulation characteristics of consonants in post school-aged patients with articulation disorders after repaired cleft palate and explore the training methods to improve the phonetic intelligibility(PI)of patients and provide reference for clinical speech treatment.MethodsThe study was conducted from February 2017 to December 2018 at speech therapeutic room in a first-class tertiary hospital in Zhengzhou.246 post school-aged patients aged 7 and over with articulation disorders after repaired cleft palate who met the inclusion criteria were selected as research objectives.Patients were given speech assessment by using Computer Speech Lab(CSL)according to speech intelligibilitytest table of Hua xi oral hospital,for analyzing the articulation characteristics of abnormal consonants.76 post school-aged patients who received speech training in speech therapeutic room in a first-class tertiary hospital in Zhengzhou during January 2018 to December2018.They were recorded respectively using the Chinese speech intelligibility test table before and after speech training.At the end of the training,we calculated the value of Phonetic Intelligibility(PI),PI increased value and evaluated the training effect.SPSS21.0 statistical software was used for statistical analysis.The type of experimental design of the first part of the study was descriptive research and that of the second part was self-paired design and paired t-test was performed for the speech training effect,α=0.05.Results246 cases of post school-aged patients with articulation disorders after repaired cleft palate,the proportion of school-age,adolescence,adulthood patients was respectively:53.3%,21.5%,25.2%.Groups of misarticulation were involved in all consonants with different places of articulation.The main erroneous articulation was apical articulation disorders.And the highest percentage of error was blade-palatal,the lowest percentage of error was labiodental in each group.The error forms of articulation disorders with highest incidence were substitution of /b/ and /p/,omission of /g/,omission of /g/ among three groups,the incidence of which were38.9%,66.0%,48.4% respectively.The error types of articulation were omission,weak pressure,Compensatory articulation(CA)and substitution.The incidence of omission was almost the same in each group.But the incidence of substitution was decreased with the increase of age.The incidence of CA and weak pressure were significantly increased with the increase of age.The highest incidence of substitution lay in school-age group,which was 96.2%.The highest incidence of CA and weak pressure lay in adult group,which were 93.5% and 61.3% respectively.76 patients with repaired cleft palate were improved from(40.93±15.82)%before speech treatment to(89.82±5.03)% after treatment.The speech intelligibility was significantly improved through speech training and the difference was statistically significant(t=36.54,P<0.001).The difference among three groups was not statistically significant(H=1.61,P=0.446).Comparison of the time needed to corrected target of error consonant among three groups:the time showed a decreasing with the age and the difference was statistically significant(H=37.639,P<0.001).The difference of the time needed to master the target consonant was statistically significant(H=53.915,P<0.001)and showed a trend of increasing with age.ConclusionThe articulation characteristics of consonants in post school-aged patients with articulation disorders after repaired cleft palate are complex and mainly show apical articulation disorders.Omission,weak pressure,CA and substitution are main error types of articulation.Phonetic characteristics is specific in patients in different ages.Targeted speech training methods are established according to the personalized articulation characteristics of post school-aged patients,and the effect of speech training is significant. |
