| Backgroundfemoral intertrochanteric fracture is a common clinical disease,occurs in the elderly population,and with China’s aging population is increasing,resulting in intertrochanteric fracture incidence showed a rising trend and young.Due to the older patients,and patients often accompanied by neurological disorders,poor balance,leading to a higher incidence of fractures in the elderly.Data show: femoral intertrochanteric fracture for hip fracture patients caused by falls 31.%-51%.At the same time,the elderly’s ability to repair the trauma is poor,the loss of bone mass,combined with osteoporosis,resulting in intertrochanteric fracture mortality in15%-20.0%,seriously affecting the health of the elderly in china.At present,the clinical treatment of intertrochanteric fractures is relatively more,including: PFNA internal fixation,artificial hip replacement,etc.,resulting in the clinical treatment of patients who choose to have a larger controversy.In this study,patients with intertrochanteric fractures were treated with artificial hip joint replacement and PFNA internal fixation for the treatment of femoral intertrochanteric fractures.ObjectiveTo investigate the clinical effect of artificial hip replacement and PFNA internalfixation in the treatment of intertrochanteric fractures.Methodsa total of 140 patients with intertrochanteric fracture were selected in our hospital from January 2014 to-2016 in.The patients were retrospectively divided into the control group(n=70)and the observation group(n=70).The control group was treated with PFNA internal fixation.The observation group was treated with artificial hip replacement therapy by ELISA in 2 groups before and after treatment of interleukin-6(IL-6),interleukin-10(IL-10)and tumor necrosis factor-a(TNF-a)level;Harris scale was used to evaluate the the joint function of patients,using SF-36 scale to evaluate the quality of life of the 2 groups before and after treatment,including 2 groups of indicators related to the operation,record operation time,bleeding volume,postoperative drainage volume and bed time,the collected data are used SPSS18.0 software for processing.Resultsin the observation group,operation time,bed time,shorter than the control group(P<0.05);the observation group blood loss,postoperative drainage,more than the control group(P<0.05);the Harris score had no significant difference between the 2groups before operation(P>0.05);the observation group after 1 months and 3months,Harris 14 d the postoperative score was higher than the control group(P<0.05);the 2 groups before treatment SF-36 score had no significant difference(P>0.05);the observation group after treatment,physiological function,physiological function,body pain,general health,vitality,social function,emotional function and mental health scores higher than the control group(P<0.05);no the 2groups before treatment,TNF-a,IL-6 and IL-10 level difference(P>0.05);the observation group 3 months after treatment,TNF-a,IL-6 and IL-10 levels lower than the control group(P<0.05);complications in the observation group after treatmentThe birth rate was 2.86% in the control group and the incidence of complications in the 2 groups was statistically significant(P<0.05).ConclusionProvide a new method for the treatment of femoral intertrochanteric fractures in the elderly patients in mastering the indications of the premise,especially for elderly patients with basic diseases,not long-term bedridden intertrochanteric fractures with artificial hip replacement,can significantly shorten the patient bed time,improve the patients in the early stage the joint function and life quality,shorten the healing time,can decrease the level of inflammatory factors,but treatment should choose appropriate treatment according to the actual situation of each patient,and preoperative patients to strengthen assessment,a comprehensive understanding of patients,make patients more targeted treatment. |
