Objective:To evaluate left ventricular segmental strains,global strains,left ventricular rotation,left ventricular twist,left ventricular torsion,and systolic function in patient with acute myocardial infarction after percutaneous coronary intervention by three dimensional speckle tracking imaging,and to clarify expound characteristics of early myocardial strain and torsion in early stage after PCI.Methods:30 patients performed PCI because of left anterior descending branch disease were enrolled as the case group,30 healthy middle and elder people were selected as the control group at the same time.After two-dimensional ultrasound data were obtained regularly,myocardial motion and systolic functions of patients in both groups were detected with 3D-STI,and all segmental strain,global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS),global area strain(GAS),peak basal twist(Ptw-B),peak apical twist(Ptw-A),left ventricular twist(LVtw),peak basal rotation(Prot-B),peak apical rotation(Prot-A),left ventricular rotation(LVrot),peak basal torsion(Ptor-B),peak apical torsion(Ptor-A),left ventricular torsion(LVtor)and left ventricular ejection fraction(LVEF)value were collected.Results:1.The global strain value of the case group was lower than that of the control group.2.Compared with the control group,the longitudinal strain in left ventricular segment 9/16(LS),the circumferential strain of segment 8/16(CS),the radial strain of segment 5/16(RS),area strain of segment 11/16(AS)were decreased in case group(p<0.05);the lower section mainly concentrated in the anterior basal segment,the middle section of the anterior wall,anterior wall,anterior septal apical basal segment,middle segment,anterior septal posterior septal basal segment,middle segment and posterior septal posterior septal apical segment.3.The global rotation angle of the left ventricle,the left ventricular torsion angle,and the left ventricular torsion were lower than those of the control group,the difference was statistically significant.The peak value of the systolic phase angle,the peak value of the systolic rotation angle and the systolic torque of the corresponding segment of the basal plane and the apical plane were lower than those in the normal group,and the difference was statistically significant.4.GLS,GAS GCS and GRS were correlated with LVEF in cases(r=0.819,r= 0.897,r= 0.807,r=0.862,P<0.0001).5.The peak value of the global rotation angle,the peak value of torsional angle and the peak torsion of the left ventricle.have a positive correlation with LVEF(r=0.834,P<0.0001;r=0.779,P<0.0001;r=0.839,P<0.0001),Conclusion:1:3D-STI can quantitatively analyze the changes of left ventricular global and regional myocardial strain in patients with AMI after PCI.2:The early postoperative left ventricular rotation,torsional motion changes could be found in patients with AMI by 3D-STI after PCI,the globe and local myocardial torsion,rotation values were decreased.3.Left ventricular strain,left ventricular rotation,left ventricular rotation and left ventricular torsion were correlated with left ventricular systolic function.
