Objective:To explore the expression of HMGB1 and T lymphocyte subsets in children with MPP;to investigate the regulatory effect of compound glycyrrhizin on T lymphocyte subsets and HMGB1;to observe the effective and side effect of compound glycyrrhizin on MPP.Methods:We enrolled clinical diagnosed MPP inpatients who were treated in Pediatric Respiratory Department of the First Bethune Hospital of Jilin University during 2015 January to 2016 December as the research object.All the patients were randomly divided into three groups according to treatment drugs:the countrol group,compound glycyrrhizin group,hormone group,each group include 20 cases.All cases were given the basic treatment such as antifebrile,reduce phlegm and atomization.Control group was given azithromycin,glycyrrhizin group was given compound glycyrrhizin besides control group,hormone group was given methylprednisolone besides control group,The course of treatment was 10 days.Healthy children during the same period were selected as the blank group.T lymphocyte subsets were performed by flow cytometry,HMGB1 was determined by ELISA.To observe the clinical efficacy and side effects of fever,shortness of breath,nasal incitement,cyanosis and pulmonary rales in 5 days,comparison of clinical efficacy and side effects after treatment.We analyzed all the data by the software SPSS 21.0 statistically.Results:1.Comparing T lymphocyte subsets between MPP group and blank group,CD4+and CD4+/CD8+decreased,CD8+increased,P<0.05,there is statistically significant differences.2.After the reatment of compound glycyrrhizin group and hormone group,CD4+increased,CD8+decreased,CD4+/CD8+increased,P<0.05,significant difference,but compound glycyrrhizin group of CD4+/CD8+close to the level of normal children.3.Comparing HMGB1 between MPP group and blank group,there is elevated levels of HMGB1,P<0.05,it had statistically significant in MPP group.After treatment,HMGB1 decreased,P<0.05,the difference was significant,but still higher than the blank group,it had not statistically significant.4.Comparing HMGB1 between compound glycyrrhizin group and hormone group after treatment,they were bigger than the control group,it had statistically significant differences(P<0.05),but Comparing with the hormone group,it had not statistically significant differences(P>0.05).5.By the comparison with each group clinical effect,it show that as follows:Firstly two treatment groups are higher than regular treatment group(P<0.05).Secondly glycyrrhizin group is lower than hormone group(90%),it had not statistically significant differences(P<0.05).The side effects of the group were significantly lower than the hormone group(20%),P<0.05,the difference between the two groups was obvious.Conclusion:1.There is decreased levels of CD4+cells,elevated levels of CD8+cells and decreased levels of CD4+/CD8+in the acute phase in the children of MPP.These results suggest that T lymphocyte subsets disorder is involved in the pathogenesis of MPP.2.After the reatment of compound glycyrrhizin,there is elevated levels of CD4+and CD4+/CD8+cells,decreased levels of CD8+cells,and the levels of CD4+/CD8+is closed to normal children.Results showed that compound glycyrrhizin can regulate the deviation of T lymphocyte subsets.3.MPP increased in the acute phase of HMGB1 and decreased after treatment,but was still higher than that in normal children.These results suggest that HMGB1can be used as an inflammatory factor in the acute phase of the disease,and it can be used as a late inflammatory factor to predict the change of the disease.4.The level of HMGB1 decreased after treatment with compound glycyrrhizin was closed to the hormone,which was significantly higher than that in the control group,indicating that compound glycyrrhizin could be used as HMGB1 inhibitor in children with MPP.5.In children with MPP,the clinical efficacy of compound glycyrrhizin was similar to that of the hormone group,but there were no side effects such as increased appetite and weight again.
