| Objetive : To explore the best means of perioperative psychological protection of children patients and to promote the degree of satisfaction of patient family members by comparing effects of different behavioral interventions on the reduction of perioperative anxiety of children patients at different ages.Material and Methods : Study 94 cases of pediatric selective operations(1-7 years old) and divide them into two groups by age- 1 to 5 years old are preschool group, 54 cases; 5 to 7 years old are school-age group, 40 cases.Separately divide the above two groups into 4 smaller groups by random number table method-A group: toy players; B group: operating room visitors; C group:the comprehensive group (combine playing toys and visiting the operating room); D group: the control group. Psychological intervention: According to the surgery schedule, anesthesiologist visits the patient the day before the surgery, to get the basic information and comprehensive condition of children patients before the surgery, to assess their temperament(EASI) and test anxiety levels in preoperative visits with the improved Yale perioperative anxiety scale(mYPAS)(T1), and to explain the anesthesia process to the family members for better communication and understanding. Implement different behavioral interventions on children patients or on their family members the day before the surgery and on the day of the surgery according to the above grouping.After inventions, test anxiety levels of the two groups at five different time points,in the anesthesia preparation room (T2) , in the operating room (T3), 6 hours after the surgery (T4) and 24 hours after the surgery (T5)-with the improved Yale perioperative anxiety scale(mYPAS). At the T3 time point, test the cooperation degree of anesthesia induction with Induction period cooperation scale (ICC) at the same time.Results: The EASI scores of preschool children patients are higher than those of school-age children patients;in the results of the tests implemnted on the selected children patients, mYPAS anxiety scores at T2 and T3 are higher than those at T1; mYPAS anxiety scores of preschool-toy players are lower than those of other preschool groups, so are the ICC scores; mYPAS anxiety scores of school-age-operating room visitors are lower than those of other school-age groups, so are the ICC scores.Conclusions:1. Proper psychological inventions before surgery can help to reduce perioperative psychological anxiety, improve the cooperation in the process of anesthesia and surgery, and promote the satisfaction degree of patient family members.2. By playing with toys and interacting with them, the anxiety of preschool children patients can be reduced or eliminated; by visiting the operating room before surgery is a more effective method in perioperative psychological protection for school-age children patients. |
