Research On Efficacy Of Topical Application With The Combination Of Chinese And Western Medicines In Promoting Healing Wound Post Anal Fistulectomy Of Rat Surface Fistula Model

Objective :Through research on efficacy of topical application with the combination of Chinese and western medicines in promoting healing wound post anal fistulectomy of rat surface fistula model,in order to use this way to clinical,promot healing wound post anal fistulectomy of patiens.Methods: Through embedding the Spring gauze,to make a model of rat's surface fistulas.Firstly,successfully model rats with post anal fistulectomy;and then randomly and equally divide those rat models into four Groups with 20 rats in each group while being feeded individually.Those four groups are one group D with combination of Dragon's Blood Capsules and Amikacin Lotion,and group A – the blank control group with normal saline(NS),and group B with Amikacin Lotion,and group C with Dragon's Blood Capsules.Give a corresponding once-per-day treatment to mounds of all these rats separately and simultaneously from the second day after successful anal surgery.Randomly select 5 rats from each group separately on the third,seventh and fourteenth day after the surgery,use HE Staining to observe wound inflammation situation of wound tissues,use Masson trichrome staining to observe collagen-fiber hyperplasia in granulation tissues,and use CD34 staining to count new blood vessels inside granulation tissues.For the rest,separately record and analyze the time of wound healing and epithelization and wound healing rates.Result: By perspectively comparing records of group D with those of group A,group B and group C on the third day,the seventh day and the fourteenth day after the surgery,it comes to the following conclusions:(1)as for the wound healing rate,all are the outcome: P<0.05;(2)Through observing wound inflammation by HE staining,it can be found that compared with other three control groups on the third day after surgery,there are less inflammation and a certain amount of new blood vessels,fibroblasts and fibrous tissues hyperplasia in group D;on the seventh day after surgery,there are a large number of new blood vessels,further quantity of fibroblasts and further expanded fibrous tissues of rats in the group D p;and on the fourteenth day,there are a little inflammatory cells,a large quantity of fibroblasts and further expanded fibrous tissues,with a crust on the wound surface.(3)as for using Masson trichrome staining to count collagenous fiber,firstly comparing group A with group D and control group B on the third day,secondly comparing group A with group D,group B and group C,and then comparing group B with group D and group C,and then comparing group D with group C on the seventh day;thirdly comparing group D with group A,group B and group C on the fourteenth day,all the outcome are P<0.05.(4)when it comes to observing new blood vessels by the immunohistochemistry staining CD34,firstly comparing group A with group D,group B and c group C and then comparing group B with group D and group C on the third day,secondly comparing group A with group D,group B and group C,and then comparing group B with group D and group C,and then comparing group D with group C on the seventh day;thirdly comparing group A with group D,group B and group C and then comparing group D with group B on the fourteenth day,all the outcome are P<0.05.(5)As for the time of healing,comparing group D with group A,control group B and group C,the outcome is P<0.05.Conclusion: Topical application of Dragon's Blood Capsules together with Amikacin Lotion in infected wound can prompt healing wound and shorten healing time.The acting mechanism of which may be related to the component of the Dragon's Blood Capsules-flavonoids.Flavonoids can promote fibroblasts proliferation and thus accelerate the synthesis of collagen fiber.At the same time,Dragon's Blood Capsules have has the effects of sterilization and anti-inflammation thanks to its polyphenols components,promoting anti-inflammation in wounds.By combined with Amikacin Lotion,its anti-infection ability can be strengthened,thus reducing inflammation in wounds and providing a good environment for hyperplasia for new granulation tissue hyperplasia.In conclusion,the topical and combined application of those two medicines in wound healing following high complex anal fistulectomy can reduce secretions,improve healing rate,shorten time of healing,as well as reduce to a certain extent the recurrence rate,it iscompletely feasible.
