Hepatocellular carcinoma(HCC)is the most common liver malignancy in the world.The incidence rate is increasing in recent decades.However,most studies have shown that the expression of PD-L1 is the most important factor in the treatment of hepatocellul ar carcinoma(HCC).However,most of the studies have shown that PD-L1 is the most important factor in the treatment of hepatocellular carcinoma.PD-L1 in rs17718883,rs4143185 and rs2890658 single nucleotide polymorphisms and serum PD-L1 in many tumors have been reported,but rarely reported in the liver cancer.Therefore,the aim of this study was to investigate the relationship between serum PD-L1 and its single nucleotide polymorphisms and the susceptibility to hepatocellular carcinoma and the prognosis of patients.The subject is divided into two parts.The single nucleotide polymorphisms of PD-L1 gene in rs4143185,rs2890658 and rs17718883 were detected in the first part of 264 human serum samples(123 cases of primary hepatocellular carcinoma and 141 healthy controls).To observe the relationship between the polymorphism of these three loci and the susceptibility and prognosis of HCC.In the second part,we collected the serum from 160 patients with HCC diagnosed by the Cancer Center between February 2 010 and January 2011 compared with 160 healthy adult sera.The relationship between circulating PD-L1 and hepatocellular carcinoma was studied by ELISA.Clinical data of all liver cancer subjects,including depth of tumor invasion,lymph node metastasis,distant metastasis,TNM staging,and medical records.All patients were not treated with radiotherapy before taking the specimen.This subject was reviewed by Daping Hospital Ethics Committee.We use Excel to record data and use spss19.0 and Prism 6.0 for statistical analysis.Results: The first part of the study showed that rs2890658 AC heterozygous mutant genotype could reduce the risk of liver cancer(OR = 0.43;95% CI: 0.20-0.94;P = 0.035),rs17718883 CG heterozygous mutant genotype can also reduce the risk,There was no correlation between the rest genotype and susceptibility to HCC;(P> 0.05).When rs2890658 mutated genotype AC and CC,we found that AC + CC and AA in the median survival time: 20 m vs.17m;P = 0.042,the difference was statistically significant;The median survival time of CC,GC and GG genotypes at the rs4143185 locus of PD-L1 was 15 m,21m and 18 m,P = 0.000,The difference was statistically significant.Rs4143185 mutations in the genotype can get a good prognosis,HR <1;and P <0.05,the difference was statistically significant.In the second part,serum PD-L1 and AFP were detected in patients with hepatocellular carcinoma.Serum PD-L1(?0.67 ng / ml)was found to be risk factors for hepatocellular carcinoma.The sensitivity of serum PD-L1 protein to HCC was 77.4%,specificity of 60.6%,the accuracy of 60.63%;PD-L1 has a certain diagnostic value.Serum PD-L1 expression level is positively correlated with AFP protein.Serum PD-L1 can be used as a potential marker for the diagnosis of hepatocellular carcinoma.Part one:1.rs2890658 of PD-L1(AC mutant type)may reduce the risk of liver cancer,2.rs4143185 of PD-L1(GG homozygous mutant type)may increase the risk of liver cancer,3.The genotype status of the rs4143185 and rs2890658 loci will determine the efficacy of immunotherapy in patients with hepatocellular carcinoma,and the mutations will achieve better prognosis after anti-PD-L1 immunotherapypart two:1.Serum PD-L1 levels were lower than those in the normal control population,but serum PD-L1(?0.67 ng / ml)and AFP were risk factors for hepatocellular carcinoma.2.The sensitivity of serum PD-L1 protein in diagnosing HCC was 77.4%,the specificity was 60.6% and the accuracy was 60.63%.which is expected to have a certain diagnostic value.3.Serum PD-L1 expression level is positively correlated with AFP protein,Serum PD-L1 can be used as a diagnostic marker for liver cancer.
