BackgroundNeonatal hypoglycemia is a common metabolic disease in the neonatal period.Persistent or repeated hypoglycemia may lead to severe brain damage,leaving epilepsy,exercise or cognitive disorders,and cerebral palsy and other sequelae.Due to clinical manifestations of atypical,it was not caused enough attention by clinical first-line medical staff in clinical work.Hypoglycemia brain injury is affected by a variety of factors,such as asphyxia,jaundice,premature rupture of membranes,maternal gestational diabetes.In recent years,the application of prenatal glucocorticoid in the prevention of neonatal respiratory distress syndrome has been confirmed,yet it is still lack of clinical application;Effects of prenatal glucocorticoid on blood glucose is not clear.Especially for neonatal hypoglycemia and neonatal hypoglycemia brain injury related to the literature is also lacking.In addition,neonatal hypoglycemia brain injury cranial nuclear magnetic resonance performance and the relationship between the prognosis of children need further study.Purpose1.To investigate the related factors of neonatal hypoglycemia brain injury;To explore the value of prenatal glucocorticoid application to NHBI.2.To investigate the relationship between head MRI and prognosis in children with neonatal hypoglycemic brain injury.MethodA retrospective study was conducted to select 85 cases of neonatal hypoglycemia diagnosed in the hospital neonatal intensive care unit and neonatal department fromJanuary 2015 to December 2016.36 cases of neonatal hypoglycemic brain injury and hypoglycemia in 49 cases,those will be inputed the computer.The application of SPSS21 statistical software to statistical analysis.The measured data conform to the normal distribution and carry on the t test,measurement data((?) ± s).The median distribution was not expressed in the normal distribution,and the Mann-Whitney U test was used for comparison between groups.Enumeration data were used in percentage(%)and X~2 test.The factors influencing the neonatal hypoglycemia brain injury were analyzed by single factor logistic regression analysis,before the multivariate logistic regression analysis was performed.When P is less than 0.05,it is considered statistically significant.Through the diagnosis of neonatal hypoglycemia brain injury,the data of cranial nuclear magnetic resonance and the follow-up of children were analyzed by descriptive method.Result1.Multivariate logistic regression analysis showed,Opening milk time(P=0.019),duration of hypoglycemia(P=0.000),Referral source(P=0.014),that has significant in the neonatal hypoglycemia brain injury multivariate analysis;The X~2 test of the effect of prenatal glucocorticoid on neonatal hypoglycemia brain damage showed statistical significance(P=0.000).2.They are the close relationship between neonatal hypoglycemic brain injury head MRI signal changes in the location,range,abnormal duration and neonatal hypoglycemic brain injury sequelae.The larger the range of MRI signal changes and the longer the duration of abnormalities,that it is the more obvious in the sequelae.Conclusion1.NHBI is affected by a variety of factors.Opening milk time,low blood sugar duration and referral source those are independent risk factors.2.Prenatal glucocorticoid application can reduce the incidence of hypoglycemia braininjury.Significant promotion is significant in the scope of application(prevention of NRDS).3.The application of head MRI in neonatal hypoglycemia brain injury is of great significance,especially diffusion-weighted imaging is obvious advantages in the early diagnosis.Head MRI has a certain predictive value for the sequelae of hypoglycemic brain injury.
