Research objective:We aimed to search for randomized controlled trials of vagus stimulation in the treatment of depression published in recent years,to systematically review the efficacy of vagus nerve stimulation compared with sham therapy for the treatment of depressive disorder, for its application in the clinical treatment of patients with depression to provide theoretical supports and objective basis. Methods:Retrieved literatures in the database at home and abroad through computer from the built of the databases to March in 2017.The databases included Cochrane Library,Pubmed,CBM,CNKI,VIP database and Wanfang database, manual search of relevant information and the relevant bibliography. Two reviewers independently selected the literatures and extracted data according to established inclusion and exclusion criteria. The extracted data were analyzed by Review Manager version 5.3 software(Cochrane Collaboration,Oxford,UK).Results:A total of 4 randomized controlled trials involving a total of 408 patients with depression were enrolled in this study,222 patients were treated with vagus nerve stimulation, while the control group(n = 186)received either a placebo or sham stimulation. The results showed that the 4 studies were of high quality. The statistical analysis showed that the response rates of VNS stimulation groups was significantly higher than that of the sham stimulation groups or invalid stimulation groups(RR=0.40,95%CI[0.27,0.59],p=0.24);but the HAMD-24 score changes, there are obvious heterogeneity, descriptive analysis only, the 3 studies HAMD-24 score of VNS groups was significantly lower than that of sham stimulation group(p<0.05);The study of Rush AJ showed no significant difference between the two groups(p=0.251)in the HAMD-24 reduction rate, while the other scale scores were statistically significant(p=0.032),the experimental group was significantly better than the control group. Conclusion:VNS is a safe and economical physical therapy, which can improve the efficiency of depression and improve the depression scale score. It suggests that therapyparameters could be sited as frequency:20Hz.However,large-scale randomized controlled trials are needed.
