Study On The Effect Of Hyaluronic Acid And Hyperbaric Oxygen In The Treatment Of Vascular Embolism Caused By Hyaluronic Acid Augmentation Rhinoplasty

Objectives: 1.To explore the relationship between the using time of hyaluronidase and the therapeutic effect in the treatment of vascular embolism caused by hyaluronic acid augmentation rhinoplasty. 2.To explore the relationship between the application of hyperbaric oxygen therapy and the therapeutic effect in the treatment of vascular embolism caused by hyaluronic acid augmentation rhinoplasty.Methods: This study consists of two parts which are clinical part and experiment part,one is from September 2015 to November 2016, in the Department of plastic surgery, the Second Affiliated Hospital of Kunming Medical University, 8 patients who got vascular embolism cause by hyaluronic acid augmentation rhinoplasty were hospitalized and treated. The treatment including hyaluronidase injection, hyperbaric oxygen therapy and supportive treatment. The other one is an animal experiment,from December 2016 to February 2017,24 adult female IRA rabbits were bought, and rabbit ear artery embolization models were established. Rabbits’ ears were divided into six groups, group 1 is control group , 2,3,4,5 group is hyaluronidase injection group which got hyaluronidase injection at 4h,8h,24h,and 48h after embolization models established,group 6 is hyperbaric oxygen treatment group. On the first day,the seventh day and the fourteenth day after embolization, collecting rabbit ears image data,rabbit ears temperature and oxygen saturation at the end of the rabbit ears.Collecting rabbit ears necrosis area data and tissue samples on the fourteenth day,The data were analyzed statistically.Results:In the clinical part, 8 clinical treatment of patients who caught vascular embolism caused by hyaluronic acid augmentation rhinoplasty were cured, some with scars. In the experiment part, the experimental rabbit ear necrosis area has statistically significant differences (P<0.05) on fourteenth day after the embolism ;All of the experimental groups of rabbit ear necrosis area has growth trend; The necrosis area between groups 4 and groups 5 has no statistically significant differences; The necrotic area between Group 1 and other groups has statistically significant differences(P<0.05).Conclusion(s): Vascular embolism caused by hyaluronic acid augmentation rhinoplasty comes out ,in 48 hours local injection of hyaluronidase can be achieved significant therapeutic effect,the sooner the better. The use of hyperbaric oxygen therapy on vascular embolism caused by hyaluronic acid augmentation rhinoplasty has a certain therapeutic effect.Hyperbaric oxygen therapy can be an adjunctive therapy in clinical.
