Objective To investigate the safety and clinical efficacy of transurethral 160W green laser prostate hyperthermia in the treatment of high risk benign prostatic hyperplasia by comparing the treatment of 160W PVP and 80W PVP and TURP in the treatment of high risk benign prostatic hyperplasia.Methods A total of 180 elderly patients with benign BPH who had one or more important organ comorbidities were treated in our hospital from September 2013 to March 2016 were selected. Randomly assigned 3 groups, 60 patients in each group. 160W PVP group: the application of 160W green laser system by transurethral vaporization of the prostate, 80W PVP group: 80W green laser system using transurethral vaporization of the prostate, TURP group: transurethral resection of the prostate. Hemoglobin and serum sodium were measured before and after surgery. The patients were followed up for 6 months. The Qmax . PVR, IPSS.QOLwere measured before and 6 months after operation , and recorded postoperative adverse reactions, indwelling catheterization time, hospitalization time and the occurrence of recent complications.Results Surgery were successfully completed, followed up for 6 months, no less of patients.The mean age of the three groups of patients and the mean volume of the prostate were not statistically significant (P> 0.05). Three groups of patients before and after surgery TPSS, Qmax,PVR, QOL observation index were statistically significant differences (P <0.05). The changes of hemoglobin before and after operation and the average operation time were statistically different(P <0.05). There were no significant differences in the changes of serum sodium before and after operation between 160W PVP group and 80W PVP group, and there was no significant difference between postoperative catheterization time, hospitalization time (P> 0.05). but there was significant difference compared with TURP group (P <0.05) Three groups were not blood transfusion, no electrical resection syndrome, no prostate perforation. Postoperative incontinence,secondary bleeding, follow-up period of efficacy satisfactory.Conclusions 160W PVP and 80W PVP compared with TURP can effectively improve the quality of life, reduce the lower urinary tract symptoms of BPH patients, the treatment effect is quite. At the same time ,PVP relative to TURP has the advantages of less bleeding, with high security, especially the new generation of high-power 160W PVP for the treatment of benign prostatic hyperplasia, fewer bleeding, higher efficiency, faster operation during the operation , especially suitable for complicated medical diseases , poor tolerance of patients with high-risk BPH.
