Objective:To evaluate the efficacy and safety of160W high power greenlight laserin the treatment of benign prostatic hyperplasia(BPH)with prostate volume≥80ml.Methods: From June2013to December2013a total of80cases of BPH patients withprostate volume≥80ml and required surgical intervention were admitted to the departmentof urology of the Second Affiliated Hospital of Soochow University. Patients wererandomly assigned to two groups with40patients in each group include the PVP group,in which patients were treated with160W high power greenlight laser, and the TURPgroup, in which patients were treated with traditional transurethral resection of the prostate(TURP). Operation time, changes of hemoglobin and serum sodium concentration,catheter indwelling time, and hospitalization time were recorded. International prostatesymptom score(IPSS),quality of life score (QOL),maximal urinary flow rate (Qmax),postvoid residual (PVR) of the patients Pre-and3months post-operation were alsorecorded. In addition, complications were monitored.Results: All the operations were successfully performed. Mean operation time of PVPgroup was (70.5±8.2)min. while TURP group was (72.5±7.0)min,there was nodifferences between the two groups(P>0.05). Mean hemoglobin concentration decreasepre-and post operation of PVP group was(3.5±1.1)g/L, while TURP group was (15.5±5.2)g/L;Mean serum sodium concentration decrease pre-and post-operation of PVPgroup was (1.67±0.70) mmol/L, while TURP group was (4.43±1.14), mmol/L, thedifferences were statistically significant(P<0.05).Mean post-operative catheter indwellingtime of PVP group was (3.2±1.0) d, while TURP group was (5.0±1.5) d; Meanpost-operative hospitalization time of PVP group was (4.0±0.8) d, while TURP group was(6.5±1.0) d, the differences were statistically significant(P<0.05).Complications: after three months follow-up of post-operation, mild urinary frequency and urgency was seen in4cases, mild dysuria was found in4cases, gross hematuria and dysuria were not occurred inPVP group; In TURP group, mild urinary frequency and urinary were seen in5cases,mild dysuria was appeared in3cases, gross hematuria was found in1case, and one caseoccurred dysuria. Incontinence and urethral stricture were seen in none of patients in twogroups. There was no significance about these indicators between the two groups (P>0.05).Compared with Preoperation,IPSS, QOL, Qmax and PVR were all improved significantlyin both groups at three months after operation (P <0.05), but no significant difference wasfound in the above markers between two groups (P>0.05).Conclusion: The domestic160W greenlight laser is safe and effective for thetreatment of BPH with large prostate volume compared with TURP, with lessintraoperative bleeding, shorter catheter indwelling time and hospitalization time...
