| Objective: The aim of this study is to investigate the factors that influence the early recurrence and metastasis of hepatocellular carcinoma (HCC) after hepatectomy. For those who have risk factors, timely and appropriate clinical intervention may be beneficial to reduce postoperative recurrence and metastasis rate, prolong the disease-free survival time and improve the prognosis of the disease.Methods: From January 1th, 2007 to January 31th, 2016, retrospective analysis of totally 68 cases of HCC patients who accepted surgical resection and confirmed by postoperative pathology were were done in Hepatobiliary and Pancreatic Surgery Ward in the Second Affiliated Hospital of Kunming Medical University. According to whether cases recurred in a year or not, patients were grouped as early recurrence group (n=33) or no recurrence group(control group, n=35). 48 possible factors that may influence early recurrence and metastasis after hepatectomy were enrolled in this study .The follow-up results and the clinical data collected were analyzed by SPSS 21.0 software. The univariate analysis was performed by chi-square test, t-test. Those significant factors in univariate analysis were analyzed in multivariate analysis by binary logistic regression analysis to screen independent factors of early recurrence and metastasis.Results: 1 .Till January 31th, 2017, we enrolled 72 cases in this study, 4cases failed to follow-up, and the rate of loss to follow-up was 5.56%.2. Univariate analysis showed that tumordifferentiation, operation time,vascular tumor thrombus, postoperative adjuvant therapy, preoperative serum AFP level were significantly different in the two groups.3. Multivariate analysis indicated that vascular tumor thrombus were not an independent risk factor or protective factor of early recurrence or metastasis after liver resection,and poor tumor differentiation,long operation time,preoperative serum AFP level (AFP >400ng/ml) can independently affect the early recurrence or metastasis of postoperative patients, postoperative adjuvant therapy were protective factors.Conclusion: Poortumor differentiation, long operation time, preoperative serum AFP level (AFP > 400ng/ml) are independent risk factors that can affect the early recurrence or metastasis of postoperative patients,and postoperative adjuvant therapy are protective factors.After hepatectomy for patients who have risk factors above,we suggest close follow-up and adjunctive therapy when necessary. |
