Patients With Acute Myocardial Infarction After Interventional Exercise Rehabilitation Scheme Of Construction And Empirical Research

Abstract/Summary:
ObjectiveBy applying the theories of evidence-based medicine,we focused on the patients who had received interventional therapy because of attack of acute myocardial infarction,and retrospectively analyzed the effects of cardiac rehabilitation and exercise rehabilitati-on of these patients,thus identifying the problems remaining in this rehabilitation system.Based on the situation of our country,we referred to the guidelines of caring the patient with coronary heart disease,and put forward a Tai Chi exercise rehabilitation program by literature retrieval and expert discussion.We further validated and modulated this program by empirical research,thus constructing an “admission,discharge and post-discharge” continual exercise program.This study aimed to(1)develop a scientific,formal and continual exercise rehabilitation system by affording theoretical basis and clinical results,(2)improve the patients’ participation and compliance in cardiac rehabilitation procedure,(3)improve the life quality of patients,and(4)reduce medical cost.MethodLiterature retrieval: we researched the literature on the cardiac rehabilitation and exercise rehabilitation for patients suffered from acute myocardial infarction,particularly international guidelines,systematic reviews and multicenter clinical trials.Expert discussion: experts from department of cardiology,traditional Chinese medicine,rehabilitation and nursing discussed and developed an exercise rehabilitation program for the patients who had received interventional therapy because of acute myocardial infarction.Empirical research: control group consisted of 29 patients admitted to department of cardiology in a tertiary referral center during July 2015 and December 2015,who had received interventional therapy because of attack acute myocardial infarction.Experimental group consisted of 30 patients admitted to department of cardiology in a tertiary referral center during January 2016 and June 2016,who had received interventional therapy because of attack acute myocardial infarction.Control group received general nurse care,and experimental group received exercise rehabilitation care system.Cardiac function,quality of life,daily competence and psychological concordance of both groups were evaluated and recorded.ResultsAccording to the analysis of the literature retrieved,the effectiveness of Tai Chi exercise rehabilitation program had been validated in several studies worldwide,however,the applying of this program for patients with acute myocardial infarction was still controversial.Based on this phenomenon,we analyzed and eliminated some dangerous sections of Tai Chi exercise rehabilitation program,and developed the “Cloud Hands Tai Chi Series”,which consisted of Cross Hands,Tuning Body,Cloud Hands and Kick.Our Tai Chi exercise rehabilitation program include 4 parts,named as a rehabilitation program during hospitalization,a rehabilitation program in short term after discharge,a rehabilitation program in long term after discharge and a checking program to evaluate the implementation.Empirical research implied that the cardiac function,quality of life,psychological concordance and daily competence were significantly different between two groups(P < 0.05),particularly,6 minutes-walking distance,LVEF,and Pro BNP,stem cell mobilization,quality of life,psychological concordance and daily competence were more significant(P <0.05).However,the difference in LVEDD were not significant.ConclusionAnalysis of literature retrieved implied the remaining problems in cardiac rehabilitation and exercise rehabilitation for patients with acute myocardial infarction,based on which we constructed a Tai Chi exercise rehabilitation program,and modulated this program by expert discuss.Empirical research manifested the effectiveness of this program on improving the cardiac function,quality of life,daily competence and psychological concordance.Tai Chi exercise rehabilitation program can effectively improve the cardiac function and quality of life of patients who received interventional therapy because of acute myocardial infarction,and promote the effect of cardiac Rehabilitation.
