OBJECTIVE: To study the condition of HBV S gene and X gene mutations and the relationship between HBV S gene,X gene mutations and cancerous,development of primary liver cancer(PLC)in Fu Sui county of Guang Xi.METHODS: A 1:1 matched case-control study was designed.30 new cases of PLC were selected from the PLC high risk group of Fu Sui(HBs Ag consecutively and HBV DNA positive from 2011 to 2015)and matched 30 controls of non-cancer were selected from this group.Collected all of their serum from 2011 to the year of diagnosed.And the DNA sequence of the S gene and X gene determined by direct sequencing following nested-PCR.The results were analyzed by chi-square test,conditional logistic regression and dynamic observation.RESULTS:1.The results of chi-square: S gene: T140 A,T140C,T300 C and G620 A had difference of mutation rate between case and control.X gene: G1721 A,T1724C and T1753 C had difference of mutation rate between case and control.The results of logistic analysis : T140C(OR=0.484)was associated with an reduced risk of PLC.T300C(OR=2.911)was associated with an increased risk of PLC.2.The results of mutation dynamic observation showed that all the HBV gene mutations occurred in the early years,and most of them kept stable mutants.T140 C,G1721A and T1753 C were reverse mutation.And the T140 C mutation was found in the case group,and this site was negatively correlated with the occurrence of PLC.The results of longitudinal analysis showed that A1762T/G1764 A double mutations had difference of mutation rate buetween case and control.CONCLUSIONS:1.Mutations of HBV S gene and HBV X gene were closely related to the occurrence of PLC.Some HBV gene mutations were associated with an reduced risk of PLC.Some were associated with an increased risk of PLC.2.The mutations of HBV S gene and X gene in this study have been mutated 3to 5 years before the diagnosis of liver cancer,and remained stable after mutation.The probability of reverse mutation is small.The longitudinal analysis of A1762T/G1764 A double mutations showed that these mutations were associated with PLC.It may be applied to the preventionof the incidence of high-risk groups of PLC.
