To Explore The Value Of The Staging Of Hepatic Fibrosis In Chronic Liver Disease

Objective:Study the relationship between platelet PLT count and chronic liver fibrosis stage,and discuss the value of PLT count in predicting chronic hepatic fibrosis stage.Methods: A retrospective analysis was used to collect 138 patients with chronic liver disease.Collect healthy check-up 414 cases as control group,collecting the experimental age,sex,blood routine,liver function and liver biopsy tissue,control group in age,sex,blood routine.Liver tissue pathology with reference to the xi 'an national conference on viral hepatitis and liver disease in 2000 revision of the standard.Comparing the experimental group and control group of men and women constitute ratio,age,PLT count difference have statistical significance.The data pretreatment is carried out by using the optimal box method,Drawing experimental liver fibrosis stage and PLT count the receiver-operating characteristic(ROC)curve,select diagnosis of experimental liver fibrosis stage when the sensitivity of approximate 70%,different experimental liver fibrosis stage in corresponding to the truncated value of PLT count and with the current commonly used APRI,FIB-4 diagnostic model to compare the specificity and sensitivity.Draw different experimentalliver fibrosis stage with PLT count step curve.Results(1)the experimental group and control group in age,gender ratio difference(experimental group: 39.2±11.2 years old,105 cases of male,female33 cases,control group 39.2±8.5years,321 cases of male,female 93);Differences between the experimental group and control group PLT count(187.7±53.4x109/Lratio,283.5±63.9x109/L,P<0.01)and(2)in the experimental group fibrosis stage of PLT count respectively S0:214.0±24.8x109/L,S1:219.7±51.3x109/L,S2:191.5±48.9x109/L,S3:169.6±43.2x109/L,S4(148.6±44.5x109/L,further analysis of variance two comparison found S4 group and S0 and S1,S2 and S1 and S3 group PLT count difference was statistically significant(P<0.01),The rest of the comparing differences between two groups of PLT count no statistical significance(P>0.05).(3)the experimental group PLT count181x109/L,or less liver tissue pathology S1(6 cases)or less,or S2(36cases);181x109/L PLT st13,liver tissue pathology S1(56 cases)or less,or S2(40 cases).(4)in the experimental group PLT count and liver fibrosis of ROC curve selection on the sensitivity and specificity of close to 70% when the PLT count cutoff value of S1 = 211.5x109/L,S2 = 199x109/L,S3 = 180x109/L,S4 =173x109/L.(5)in the experimental group PLT count,APRI,FIB4 and liver fibrosis of ROC curve sensitivity approximately 70% specificity,S1(50%,50%,100%),S2(69%,50%,69%),S3(69%,63.2%,52.9%),S4(69.4%,62.9%,54.1%).Conclusion 1 PLT count,chronic liver disease patients than healthy people PLT count is low.2,in chronic liver disease patients,PLT count 181 x109 / L or less,90.3% of the patients of liver fibrosis is greater than S2.3,PLT count prediction of liver fibrosis in a patient with chronic liver disease S1,S2,S3,S4,sensitivity and specificity of approximate 70% cutoff value is: 211.5x109/ L,199x109/L,180x109/L,173 x109/L.4,PLT count can preliminary prediction of chronic liver disease patients of liver fibrosis stage.
