Objective:To observe the bone regeneration in palatal fissure after unilateral cleft lip and palate surgery in patients with unilateral cleft lip and palate(UCLP),and to explore its effect on dental arch development.Date and Methods: A total of 35 patients with unilateral cleft lip and palate were treated with plastic surgery from June 2015 to January 2017 in hospital.The patients were aged from 9 to 18 years old.20 cases of normal teeth as a control group,aged 9 to18 years old,all subjects were CT plain scan.In the experimental group,the coronal facial window images were used to check whether there was regenerated bone bridge in the palate fracture.According to the formation of regenerative bone bridge,all the subjects were divided into two groups: regenerated bone bridge group,The length and distribution of the regenerated bone in the regenerated bone bridge were measured.The width of the dental arch was measured between the two groups.The difference between the two groups was statistically analyzed.The width of the dental arch,and then there will be regenerated bone bridge formation group compared with the normal control group,no regenerative bone bridge formation group compared with the normal control group.Results:(1)Of the 35 patients with unilateral cleft palate,25 cases had maxillary palate palattes with different degrees of bone regeneration and were connected with each other to form bone bridge.There were no obvious regenerative bone formation in the remaining palatal fissure.Positive rate was 71.4%,including 16 males and 9females,regenerated bone bridge length between 5mm ~ 21 mm,regenerated bone thickness,density is not uniform;regenerated bone mainly distributed in the first molar to the first molar segment,(2)There was no recurrence of bone in the fissure of the alveolar bone and in the palate.(3)There was no significant difference in the width of the arch and posterior segment(P<0.05),but there was no significant difference between the anterior segment and the posterior segment width(P>0.05)(Table 1).Compared with the normal control group,the width of the dental arch was smaller than that of the control group(P<0.05)(Table 2).Compared with the normal control group,The width of the posterior segment and the width of the posterior segment were smaller than those in the control group(P<0.05)(Table 3).Conclusion:(1)High-resolution spiral CT can be used for three-dimensional reconstruction of the maxilla and accurate observation and measurement,which can reflect the three-dimensional morphology of the maxilla more intuitively.(2)Some unilateral complete cleft palate patients with postoperative maxillary bilateral palatal have different degrees of regeneration of bone connected to form bone bridge closed fissure,mainly distributed in the first molar to the second molar,especially the second premolar Point to the first molar far between the significant,may be associated with local soft tissue tension,scar contracture traction related.(3)The width of the dental arch of the experimental group was significantly smaller than that of the control group.There was no significant difference in the width of the middle of the dental arch of the experimental group and the posterior arch of the arch and the arch of the arch.Bone bridge distribution position corresponding to the regeneration of bone bridge is conducive to better development of dental arch shape.(4)Unilateral cleft lip and palate patients after alveolar cleft,alveolar ridge to incisor hole,palatal bone transverse segment of the cracks still exist,no obvious bone regeneration,may be related to local anatomy,the specific reasons to be Further Discussion.(5)Regenerated bone bridge formation is conducive to the development of dental arch and maxillary morphology,in the future study can further explore how to induce the regeneration of the palate,guidance and applied to clinical,will improve the development of patients with maxillofacial.
