Objective:To analyze the clinical effect of endoscopic surgery in the treatment of obstructive hydrocephalus and to evaluate its clinical value.Methods:The clinical data of 90 patients with obstructive hydrocephalus in the First Affiliated Hospital of Kunming Medical University from December 2011 to December 2016 were retrospectively analyzed.90 patients were divided into neuroendoscope group 47 cases and ventriculoperitoneal shunt group(VPS group)43 cases.All patients were followed up,the operation time,hospitalization time,effective rate,KPS score,postoperative complications and mortality were compared between the two groups.Results:First,neuroendoscope group:the average operation time was(90.09 ± 32.34)minutes,the average hospitalization time was(20.40±7.91)days;VPS group:the average operation time was(117.60±41.74)minutes,the average hospitalization time was(27.33±17.40)days;the average operation time and hospitalization time in neuroendoscope group is shorter than that in VPS group,and the difference was statistically significant(P<0.05).Second,the neuroendoscope group effective rate was 100%,the VPS group effective rate was 83.72%,The effective rate in neuroendoscope group is higher than that in VPS group,and the difference was statistically significant(P<0.05);The KPS score in neuroendoscope group was(97.66±14.63),and KPS score in VPS group was 63.95±36.98).The KPS score in neuroendoscope group is higher than that in VPS group,and the difference was statistically significant(P<0.05).Third,the postoperative complications were compared between the two groups.The incidence of intraventricular hemorrhage,ventriculoperitoneal shunt obstruction and intracranial infection in the endoscopic group were lower than that in the VPS group,the difference was statistically significant(P<0.05).Fourth,the mortality rate was compared between the two groups.The mortality rate of endoscopic group was 2.13%,the mortality rate of VPS group was 18.60%.The mortality rate in endoscopic group was lower than that in VPS group,the difference was statistically significant(P<0.05).Conclusion:The application of endoscopic surgery in the treatment of obstructive hydrocephalus can not only solve the problem of cerebrospinal fluid shunt,but also can deal with some primary causes.Endoscopic surgery is superior to VPS in improving the clinical efficacy,improving the quality of life of patients,reducing postoperative complications,and reducing mortality.However,endoscopic surgery can not completely replace VPS surgery.It is consistent with the modern minimally invasive and accurate treatment concept.
