The Handing Strategies Of Petrosal Vein In Microvascular Decompression With Primary Trigeminal Neuralgia

Objectives:To observe the morphological characteristics of petrosal veins and the relationship with surgical approach to explore the management of petrosal vein in microvascular decompression for primary trigeminal neuralgia.Methods:The clinical data of 69 patients with trigeminal neuralgia were treated with microvascular decompression from June 2014 to June 2016 in The Hospital of Kunming Medical University were analyzed retrospectively,Use the statistics methods to comparative evaluate the management of petrosal vein in microvascular decompression for primary trigeminal neuralgia.including the clinical symptoms of the 69 cases and the relationship of the surgical approach with the SPV.The surgery outcome and complications after operation were also be.summarizedResults:The 69 primary trigeminal neuralgia patients were found the petrosal veins.In 22 patients,the surgical approach was obstructed or trigeminal nerve comperession by the trunk of the superior petrosal veins,and which should be cut in 10 cases.In 34 patients,the surgical approach was obstructed or trigeminal nerve comperession by the branches of superior petrosal veins,and which should be cut in 17 cases.Trigeminal nerve comperession by Trans pon vein and Mid cerebellar ped vein in 1 case.The surgical approach had no sbstruction form the superior petrosal vein in 13 cases.In our study,the superior petrosal vein was cut in 27 cases,which had temporary hearing disturbance in 5 cases,facial numbness 3 cases.cerebellar infarction in 1 case;One patient encountered peduncular hallucinosis and one patient dead.The superior petrosal veins weren't cut in 42 cases,which had temporary hearing disturbance in 4 case,facial numbness 2 case.Conclusions:As one of the main draining vein from posterior cranial fossa,superior petrosal vein has a close relationship with trigeminal nerve.So in micro-vascular decompression for trigeminal neuralgia the surgical approach may be obstructed or trigeminal nerve comperession by the trunk or branches of the superior petrosal veins.The treatment of superior petrosal vein is a significiants step in microvascular decompression for trigeminal neuralgia and also an significiants step to decrease the complication after operation.
