Objective: observe the effect of low purine diet combined with sodium bicarbonate on uric acid,and observe its influence on blood pressure and serum sodium after applied.Method: 90 subjects were randomly divided into three groups of A,B and C.In A group,low purine diet was as the compared group.In B group,low purine diet was combined with sodium bicarbonate 0.5 TID.In C group,low purine diet was combined with sodium bicarbonate 1 TID.After 4 weeks,the change of blood uric acid,urine PH,blood pressure in consulting room,ambulatory blood pressure,and serum sodium in three groups was compared.Experimental Result:1.After treatment,In the A group,the uric acid decreased statistically significant(490.93±65.23vs445.36±66.27,P<0.05).In the B group,the change of uric acid was statistically different(497.00±73.76vs436.26±68.45,P< 0.05).In the C group,the uric acid decreased significant after treatment(495.82±71.38vs414.82±54.03).There were statistical differences in the three groups.The result of group C is superior than group A,the difference was statistically significant.There was no significant difference between C group and B group.2.urine p H value: in A group,the change of PH was not obvious(5.79±0.91vs6.00±0.39,P>0.05).Patients in B group,the PH rose(5.40±0.76vs6.23±0.46,P<0.05),The difference was statistically significant.In C group,the PH rise as well(5.65±0.86vs6.38±0.28,P<0.05).The difference was statistically significant.3.Blood pressure in consulting room: in A group,systolic blood pressure rose after treatment(123.10±14.62vs131.90±11.66,P<0.05).Diastolic pressure had no statistical difference between before and after treatment(82.40±9.78vs82.00±7.33,P > 0.05).In B group,systolic blood pressure between before and after treatment(129.30±10.87vs131.07±10.37,P>0.05),There was no statistical difference.Diastolic pressure before and after treatment(82.70±9.80vs83.13±9.13,P>0.05),There was no statistical difference.In C group,systolic blood pressure between before and after treatment(125.60±12.82133.10±13.36,P>0.05),There was no statistical difference.Diastolic pressure had no statistical difference as well(82.60±8.49vs83.00±7.35,P>0.05).4: 24 hours Ambulatory blood pressure: in A group,systolic blood pressure between before and after treatment(126.68±12.36vs126.93±11.36,P > 0.05)There was no statistical difference.Diastolic pressure had no statistical difference between before and after treatment and after treatment(80.99±4.88vs81.10±5.25,P>0.05).In B group,systolic blood pressure between before and after treatment(131.20±9.08vs130.39±8.71,P >0.05),There was no statistical difference.Diastolic pressure before and after treatment(82.87±5.79vs82.51±6.47,P>0.05),There was no statistical difference.In C group,systolic blood pressure between before and after treatment(128.88±12.37vs128.90±11.99,P>0.05),There was no statistical difference.Diastolic pressure had no statistical difference as well(81.87±6.23vs82.44±6.44,P>0.05).5: Blood sodium: all of the three groups had no significant change after treatment.(140.64±2.27vs140.57±0.94,140.07±1.71vs140.40±0.99,141.18±2.16vs140.76±1.15,P>0.05).Conclusion and Significance: Blood uric acid could be reduced effectively by sodium bicarbonate combined with low purine diet.But the level of uric acid was insufficient.Dosage should be adjusted according to the urine ph.There was no influence on blood pressure and blood sodium.
