Background: Weight changes is an important factors affecting the pathogenesis of chronic diseases,and its impact on the hypertension and disease relatively lagging behind,from the weight began to significantly increase to the emergence of the corresponding disease may take several years or even decades,weight changes in early adult will increase the risk of chronic diseases such as hypertension,diabetes in old age.Especially for people living in a country that has go through reform and opening,living condition and lifestyle have significant changes,improvements in living conditions lead to changes in body weight,which may increase the risk of developing hypertension and diabetes in later years.At present,there are few studies for those.So our study based on the baseline data of Guangzhou Biobank Cohort Study(GBCS),to explore the correlation between weight changes during early and middle adulthood and the age of weight change and hypertension and Type 2 diabetes in old age.Methods:Based on the "Guangzhou Biobank Cohort Study"(GBCS)platform,28926 Guangzhou residents aged ?50 were selected,With detailed questionnaires,information on general demography indice,weight change after 18 years of age,the age of apparent weight gain,etc.The relationship between weight change after adulthood,body mass index,systolic blood pressure,diastolic blood pressure,mean arterial pressure,pulse pressure difference and fasting blood glucose were analyzed by covariance analysis.multivariate Logistic regression model was used to analysis the correlation between body weight changes during early and middle adulthood,age of keeping the most heavy weight and chronic diseases in old age after adjusting forage,smoking,alcohol consumption,physical activity,education level,occupation,district of residence and body mass index.Results: The mean age of the participants were 64.3(SD 6.7)years old in male and61(SD 7)years old in female.The prevalence of hypertension was 46.3% for men and42.6% for women,respectively,and the prevalence of diabetes was 13.0% and 13.6%for men and women,respectively.Body weight change after adulthood,body mass index are closely related to systolic blood pressure,diastolic blood pressure,mean arterial pressure,pulse pressure difference.The influence on systolic blood pressure,diastolic blood pressure,mean arterial pressure has an interaction for male,and on diastolic blood pressure,mean arterial pressure effects have an interaction for female.The risk of hypertension increased with weight gain during early and middle adulthood both in men and women(P<0.01 for trend),Comparing with those whose body weights were essentially the same,those weight gain above 20 Kg during early and middle adulthood had the highest risk of hypertension,the risk ratios were1.92(1.55,2.38)in male and 1.49(1.29,1.73)in female.The age of keep the heaviest weight is also associated with the risk of hypertension,Those reached the highest weight between 40 to 49 years old had the highest risk of hypertension.The risk ratios were 1.83(95%CI: 1.17,2.85)in male and 1.69(1.41,2.03)in female.Weight loss in adulthood was only associated with hypertension in later years in women.The risk of diabetes increased with weight gain during early and middle adulthood in both men and women(both P values for trend <0.01),Comparing with those with stable body weight,those who gained above 20 kg during early and middle adulthood were associated with the highest risk of diabetes(OR 2.84(95% CI: 2.02,399)for men and3.30(95% CI: 2.61,4.17)for women).The age of keep the heaviest weight is also associated with the risk of diabetes,those reached the highest weight from 40 to 49 years old were associated with the highest risk of diabetes,with odds ratios(ORs)being 4.32(95% confidence interval(CI): 1.72,10.8)for men and 3.29(2.41,4.49)for women.Weight loss in adulthood was associated with self-reported,but not new diagnosed,diabetes in later adulthood in both men and women,has nothing to do withthe new medical diagnosis of diabetes.Conclusion: Body weight gain during early and middle adulthood may increase the risk of hypertension and type 2 diabetes in later years,and the risk of illness increased with weight gain during early and middle adulthood,especially weight gain in the age of 40 to 49 years was associated with the increased risk of hypertension and diabetes,The detrimental effect of obesity on hypertension and diabetes might become significant in 10-20 year.Our study suggested that prevention and treatment of obesity should be in advance,and should pay attention to 40 to 49 age group of people's health,to maintain a relatively stable weight to prevent the occurrence of chronic diseases is very esssential.
