Objectives 1 To explore the impact of blood glucose on the nature of the coronary plaque and the degree of the coronary stenosis in patients with coronary atherosclerosis.2 To explore the impact of HbA1c on the nature of the coronary plaque and the degree of the coronary stenosis in patients with diabetes.Methods 1 From March 2016 to June in the North China University of Science and Technology Affiliated Hospital,171 patients with coronary atherosclerosis who underwent 256-slice CT coronary angiography were divided into three groups according to blood glucose levels,coronary atherosclerosis group(n=82),coronary atherosclerosis with impaired glucose regulation(IGR)group(n=30),coronary atherosclerosis with type 2 diabetes mellitus(T2DM)group(n=89),and the clinical and imaging data of these patients were collected.The number of calcified plaques,non-calcified plaques and mixed plaques were counted and the stenosis degree of each vessel was counted.Compare the number of atherosclerotic plaques,the nature of plaque and the degree of coronary artery stenosis in the three groups respectively.2 From May 2016 to September in the North China University of Science and Technology Affiliated Hospital,108 patients with coronary atherosclerosis underwent coronary angiography with 256-slice CT were divided into two groups according to HbA1c levels,coronary atherosclerosis accompanied with type 2 diabetes(T2DM)group(n=56)(Hb A1c<8%),coronary atherosclerosis accompanied with type 2 diabetes(T2DM)group(n=52)(Hb A1c?8%),and the clinical and imaging data of these patients were collected.The number of calcified plaques,non-calcified plaques and mixed plaques were counted and the stenosis degree of each vessel was counted.Compare the nature of plaque and the degree of coronary artery stenosis between these groups respectively.Results:1 The number of atherosclerotic plaques in the coronary atherosclerosis group? coronary atherosclerosis with IGR group,coronary atherosclerosis with type 2 diabetes mellitus(T2DM)group was 201?123 and 252,the average number of atherosclerotic plaques in the coronary atherosclerosis group? coronary atherosclerosis with IGR group,coronary atherosclerosis with type 2 diabetes mellitus(T2DM)group was 2.48±0.94?4.10±2.02?and 4.27±1.85,the difference between the three groups was statistically significant(F=27.357,P=0.000,P<0.05).There was no significant difference between the IGR group and the T2 DM group in the average number of plaques(P=0.619,P>0.05).The rate of detection of calcified plaques,non-calcification plaques and the mixed plaques in the coronary atherosclerosis group was 60.7%,15.4% and 23.9%.The rate of detection of calcified plaques,non-calcification plaques and the mixed plaques in the coronary atherosclerosis with IGR group was 44.7%,22.0% and 33.3%.The rate of detection of calcified plaques,non-calcification plaques and the mixed plaques in the coronary atherosclerosis with type 2 diabetes mellitus(T2DM)group was 34.5%,28.6% and 36.9%.The difference between the three groups was statistically significant(?2=31.498,P=0.000,P<0.05).The rate of the detection of the characteristics in the coronary atherosclerosis group and the IGR group,the coronary atherosclerosis group and the T2 DM group,was significant(?2=7.886,P=0.020;?2=31.197,P=0.000).The rate of the detection of the characteristics in the IGR group and the T2 DM group,was no statistically significant.(?2=3.935,P=0.140)The rate of detection of mild coronary stenosis,moderate coronary stenosis and severe coronary stenosis in the coronary atherosclerosis group was 66.5%,21.3% and 12.2%,the rate of detection of mild coronary stenosis,moderate coronary stenosis and severe coronary stenosis in the IGR group was 23.0%,35.1% and 41.9%,the rate of detection of mild coronary stenosis,moderate coronary stenosis and severe coronary stenosis in the T2 DM group was 21.0%,35.6% and 43.4%.The difference between the three groups was statistically significant(?2=83.421,P=0.000,P<0.05).There were significant differences in the detection rate of coronary artery stenosis between the coronary atherosclerosis group and the IGR group,the coronary atherosclerosis group and the T2 DM group(Z=?6.462,P=0.000,P<0.05;Z=?8.719,P=0.000,P<0.05).There was no significant differences in the detection rate of coronary artery stenosis between the IGR group and the IGR group and the T2 DM group(Z=?0.320,P=0.749,P>0.05).The rate of detection of calcified plaques,non-calcification plaques and the mixed plaques in the in the T2 DM group(Hb A1c<8%)was 56.7%,20.1% and 23.2%,the rate of detection of calcified plaques,non-calcification plaques and the mixed plaques in the in the T2 DM group(Hb A1c>8%)was 30.9%,31.3% and 37.8%,the differences between the two groups were statistically significant(?2=27.886,P=0.000,P<0.05).The rate of detection of mild coronary stenosis,moderate coronary stenosis and severe coronary stenosis in the T2 DM group(Hb A1c<8%)was 52.8%,30.8% and 16.4%,the rate of detection of mild coronary stenosis,moderate coronary stenosis and severe coronary stenosis in the T2 DM group(Hb A1c>8%)was 24.7%,40.4% and 34.9%,the differences between the two groups were statistically significant(Z=?5.365,P=0.000,P<0.05).Conclusions 1 People with normal blood sugar compared with those whose blood sugar was abnormal,the number of coronary atherosclerotic plaques increased,the rate of detection of the mixed and non-calcification plaques increased obviously,the rate of detection of the calcification plaques reduced,and the coronary artery narrow degree is more serious.2 People with abnormal glucose metabolism compared with those who were diagnosed with diabetes,the extent of coronary atherosclerosis is similar.3 As for people with diabetes,with the increase of the HbA1c,the rate of detection of the mixed and non-calcification plaques increased obviously,the rate of detection of the calcification plaques reduced,and the coronary artery narrow degree is more serious.
