The Prenatal Sonographic Diagnosis And Value Assessment Of Fetal Abdominal Cystic Echo

Objective Through the analysis of fetal abdominal ultrasonographic features of cystic echo,fetal monitoring,on a regular basis to track follow-up,analysis of its prognosis and clinical outcome,summarize,organize information before and after the birth,to explore the diagnostic value of prenatal ultrasound,differential diagnosis and clinical significance,to provide clinical basis for the treatment of newborn.Methods Collection from December 2011 to December 2016 in Beijing Obstetrics and Gynecology Hospital affiliated to Capital Medical University Department of medical ultrasound prenatal ultrasound fetal abdominal cystic echo in 147 cases,of which 92 cases of fetal ovarian cysts,35 cases of congenital choledochal cysts,20 cases of congenital intestinal duplications,take prenatal sonographic,including by two-dimensional ultrasound and color Doppler and three-dimensional ultrasonography,analysising of its characteristics,comparison of changes before and after birth,comparing with pathological results of surgery,summarizing the prognosis and outcome of fetal abdominal cystic echo.Results 147 cases were included in the final study of the 141 cases,lost in the follow-up of 6 cases(lost to follow-up in 5 cases of ovarian cysts,1 cases of intestinalduplication).The study included 87 cases of prenatal diagnosis of ovarian cysts(87/141,61.7%),35 cases of prenatal diagnosis of congenital choledochal cysts(35/141,24.8%),19 cases of prenatal diagnosis of congenital intestinal duplications in(19/141,13.5%).74 cases of ovarian cysts(74/87,85.1%)regressed spontaneously,13 cases(13/87,14.9%)were treated surgically.The pathological examination results are: 5 cases(5/13,38.5%)ofsimple ovarian cysts,3 cases (3/13,23.1%)of torsion of unilateral ovarian cysts,5cases(5/13,38.5%)of benign teratomas;35 cases of congenital choledochal cysts in prenatal diagnosis,born after neonatal abdominal ultrasound examination of 27 cases of congenitalcholedochal cysts,prenatal ultrasound diagnosis coincidence rate was 77.1%(27/35,77.1%),27 cases of neonatal abdominal ultrasound for congenital choledochal cyst in 26 cases(26/35,74.3%)confirmed by pathology of congenital choledochal cysts,were born within 30 days after surgery,followed up to 12 months of birth after a good prognosis,no complications,and other indicators of transaminase were improved;1 cases(1/27,3.70%)due to surgical delay because of the death ofkernicterus.Misdiagnosis of 7 cases,including 4 cases of hepatic cysts(4/7,57.1%),2 cases of congenital biliary atresia(2/7,28.6%),1 cases of ovarian cyst(1/7,14.3%),underwent surgical treatment,1 case(1/35,2.9%)of induction,without pathology;prenatal ultrasound diagnosis of 19 cases of congenital intestinal duplication in 6 cases(6/19,31.6%)was born after surgery and pathology were intestinal duplication;11 cases(11/19,57.9%)for neonatal abdominal ultrasound diagnosis of congenital intestinal duplication,because of asymptomatic patients is still under observation,prenatal ultrasound diagnosis coincidence rate(17/19,89.5%);2 cases(2/19,10.5%)underwent neonatal abdominal ultrasound examination no cyst,considering the misdiagnosis.ConclusionsFetal abdominal cystic echo can be checked out by ultrasound in the prenatal sonographic.features of different cysts are in different parts and types,with the most common of ovarian cysts,prenatal diagnosis is more easy,the pregnancy can be regularly observed,the prognosis is good and the majority can fade;congenital choledochal cysts could be diagnosis in prenatal ultrasound,The need for timely surgical treatment after birth as soon as possible;congenital intestinal duplication of prenatal diagnosis is difficult,checking out after birth by abdominal ultrasound diagnosis of newborn,the prognosis is good,regular review,and observation of clinical manifestations,could be elective surgery.Prenatal ultrasound of fetus abdominal cystic echo diagnosis and treatment is of great clinical significance.
