BackgroundFetal lung is an important organ of the fetus,its development directly affects the quality and life of perinatal children.Pulmonary space-occupying lesions may lead to different degrees of the pulmonary hypothalami due to compression of adjacent normal fetal lung tissue,in severe cases,fetal edema can occur,and even uterine death.Therefore,it is of great clinical value to observe whether the lung is combined with space occupying lesion in fetus.In prenatal ultrasonographic diagnosis,fetal lungdysplasia,especially type ? congenital cystic adenomatoid malformation of the lung,because of similar to the echo of surrounding normal lung tissue,is mostly not obvious,and subjective judgment is relatively difficult.which further indicated the necessity of objective quantitative evaluation of fetal pulmonary echo.At present,the diagnosis of pulmonary space-occupying lesions is mainly qualitative,while quantitative analysis has not been reported in China.This study intends to quantitatively analyze and evaluate normal lung and its space occupying lesions through the measurement of lung intensity.This paper is divided into two parts.The first part is the correlation between pulmonary ultrasonic echo intensity and gestational age.The second part is the value of ultrasonic echo intensity value in the diagnosis of fetal type ? congenital cystic adenomatoid malformation of the lung.Part ? Correlation between pulmonary ultrasound echo intensity and gestational ageObjective:The purpose of this study was to quantitatively analyze and evaluate the echo intensity of normal lung by measuring the echo intensity of normal fetus at different gestational weeks.Methods:In this study,echo intensity values at the same depth of lung and liver were measured in 192 cases of normal fetuses at 16?39+6 weeks of gestation.The echo intensity of normal fetus at the same depth of lung and liver was measured and recorded by two doctors respectively.Another physician measured the echo intensity of the same fetus at the same depth of lung and liver twice,and conducted intra-group repeatability test.Liver and lung echo intensity values at different gestational weeks were recorded for statistical analysis,and the correlation between normal fetal liver,lung echo intensity and ratio of echo intensity of liver and lung was obtained.Results:1.The intra-group and inter-group repeatability of the measurement data of the two physicians were good(P>0.05).2.There was no significant correlation between normal fetal pulmonary echo intensity and gestational age(R2=0.003).3.There was no significant correlation between normal fetal liver echo intensity and gestational age(R2=0.03).4.There was no significant correlation between echo intensity ratio of liver and lung and gestational age(R2=0.02).Conclusion:Normal fetal liver and lung echo intensity values and ratio of echo intensity of liver and lung did not change with the increase of gestational weeks.Part ? The value of ultrasonic echo intensity in the diagnosis of fetal type ? congenital cystic adenomatoid malformationObjective:The purpose of this study was to evaluate the diagnostic value of ultrasonic echo intensity value in the diagnosis of congenital cystic adenomatoid malformation(CCAM ?)of type ? lungs through quantitative analysis and evaluation of CCAM ? lungs by measuring the value of congenital cystic adenomatoid malformation(CCAM ?).Methods:34 cases of CCAM ? fetus were measured and recorded in terms of echo intensity on the affected side and on the healthy side.Paired t test was used to analyze whether the difference in echo intensity between the affected side and the healthy side of CCAM ? fetus was statistically significant.Results:The difference between the pulmonary echo intensity of the affected side and that of the uninjured side was statistically significant(P<0.05).Conclusion:Pulmonary echo intensity value can be used as a safe,non-invasive and simple method to evaluate fetal abnormal lung development.It can quickly and accurately determine the presence of CCAM ? in fetal lung before delivery.
