Objective:To investigate the morbidity rate of diabetic peripheral neuropathy in patients with type 2 diabetes mellitus,and to analyze the related risk factors of diabetic peripheral neuropathy.Method: Based on our hospital medical record retrieval system,retrospective collection clinical data of 487 patients with type 2 diabetes in the Department of Endocrinology of Lanzhou General Hospital of Lanzhou military from June 2015 to December 2015.Clinical data mainly includes as follows,firstly,hospitalized patients with general information includes name,gender,age,identity number,nation,body mass index(BMI),smoking history,drinking history and family history.Secondly,information of blood glucose includes fasting plasma glucose,plasma glucose before lunch and dinner,2 hours postprandial plasma glucose,bed time plasma and plasma glucose at 3 a.m;whether there is hypoglycemia or hypoglycemia reaction,the maximum difference of blood glucose fluctuations within 24 hours in hospital at first.Thirdly,diabetes-related data includes glycosylated hemoglobin(HbAlc),fasting insulin and C peptide,2 hours after meals insulin and c-peptid.Fourthly,Other laboratory indexes includes high density lipoprotein(HDL-C),low density lipoprotein(LDL-C),total cholesterol(TC),triglyceride(TG);uric acid(UA),super oxide dismutase(SOD),urinary inhibition C(CYS-C),urine microalbumin,urinary microalbumin creatinine ratio.Lastly,complications includes diabetic nephropathy(DN),diabetic retinopathy(DR),hypertension,cardia-cerebrovascular disease,fatty liver,arteriosclerosis.Based on whether patients with diabetic peripheral neuropathy(DPN),the patients were divided into DPN group and NDPN group.Put all the information in Microsoft excel and sorting.SPSS20.0 statistics software was applied to analysis the prevalence of and risk factors.Results:(1)There were 204 cases complicated with diabetic peripheral neuropathy in the487 cases of T2 DM patients,and the morbidity rate of DPN was 41.89%.(2)The difference of age,duration of disease,HbAlc,CYS-C,SOD,2-hour C-peptide,urinary microalbumin was statistically significant in DPN and NDPN(P<0.05).(3)Female prevalence of T2 DM patients with DPN was obviously higher than that of men(P<0.05);The difference of patients with diabetic retinopathy,diabetic nephropathy,hypertension,cardiovascular andcerebrovascular disease,atherosclerosis,hypoglycemia or hypoglycemia reaction was statistically significant(P<0.05).The patients with fatty liver in NDPN more than DPN,and the difference was statistically significant(P<0.05).(4)Logistic regression analysis showed that age,course of disease,HbAlc,and hypoglycemia or hypoglycemia were independent risk factors for DPN.Hypoglycemia or hypoglycemia was the most risk factor for DPN(OR = 2.04).(5)With the increase in age and duration of diabetes,DPN morbidity rate increased.In patients with T2 DM has complicated with diabetic nephropathy or diabetic retinopathy,the possibility of DPN.Conlusion:(1)Age and duration of diabetes are uncontrollable factors in T2 DM patients with DPN,and the patients with diabetic nephropathy or diabetic retinopathy,the possibility of DPN,we should be paid attention to early screening.(2)T2DM patients with HbA1 c too high or too low can lead to increased prevalence of DPN,therefore,in the early stage of diabetes,plasma glucose control in the ideal range as early as possible is very important.In addition to avoid high blood sugar,we also should avoid hypoglycemia or hypoglycemia reaction,in case of adverse consequences that made patients permanently disabled.
