Objective: To investigate the correlation between the plasma LP-PLA2 level of patients with large-artery atherosclerosis stroke and the stenosis degree of intracranial and extravascular atherosclerosis?Method: 60 patients with LAA who were enrolled by the Department of Neurology of our hospital from December 2015 to December 2016 were taken as research objects.40 healthy people who did physical examination in the Physical Examination Center of our hospital were selected as the control group.The double-antibody sandwich enzyme linked immunosorbent assay(ELISA)was adopted to measure the LP-PLA2 level of people in the two groups.Meanwhile,their biochemical indexes such as blood glucose,blood lipid and homocysteine were measured.The two groups were examined by neck ultrasonography to check the degree of atherosclerosis,and check if there is atherosclerosis plaque or plaque nature.Head and neck CTA examination was carried out to observe the extent of vascular stenosis in head and neck.SPSS 22.0 statistical software was used to process the research data.Results:1 The smoking proportion,alcohol consumption proportion,hypertension proportion and diabetes proportion of LAA group were higher than those of control group,the difference was not statistically significant(P>0.05).The average age and male ratio of LAA group were lower than those of control group,the difference was not statistically significant(P>0.05);2 The LDL,TC,TG and Fib levels of LAA group were higher than those of control group,the difference was statistically significant(P<0.05),the HDL level of control group was higher than that of LAA group,the difference was statistically significant(P<0.05);3 The LP-PLA2 level and HCY level of LAA group were significantly higher than those of control group,the difference was statistically significant(P<0.05).4 Among the patients of LAA group,the LP-PLA2 level showed a significant increase trend with the increase in the severity of neurological deficits,and the difference was statistically significant(P=0.001).5 In the LAA group,there were 34 patients with infarction in the blood supply area of internal carotid artery system,and their LP-PLA2 level was 156.26±31.46ng/ml.There were 21 patients with infarction in blood supply area of vertebral-basilar artery system,and their LP-PLA2 level was 164.68±21.17ng/ml,the difference was not statistically significant(P=0.247);6 A comparison between the vascular stenosis at different places of subgroups in the LAA group was made.The inter-group difference was not statistically significant(P=0.695).7 The plaques of 56 patients in the LAA group were detected,11 patients had stable plaques,LP-PLA2 level was 170.59±38.16ng/ml,45 patients had vulnerable plaques,with LP-PLA2 level of 192.86±37.29 ng/ml,which was significantly higher than the LP-PLA2 level of the group without any plaque(112.38±26.17ng/ml),and the difference was statistically significant(P=0.000)8 There were 5 patients with vascular grade 0 in the LAA group,their LP-PLA2 level was 112.25±28.65ng/ml,15 patients with vascular grade 1,their LP-PLA2 level was 149.76±31.18ng/ml;32 patients with grade 2,their LP-PLA2 level was 175.38±41.24 ng / ml,and 8 patients with grade 3,their LP-PLA2 level was 185.59±39.52ng/ml,the difference was statistically significant(P=0.002).Conclusion: The results of this experiment show that plasma LP-PLA2 may be a new risk factor of atherosclerotic cerebral infarction,it is significantly associated with the severity of atherosclerotic cerebral infarction,has a predictive effect on the severity of intracranial and extravascular atherosclerosis,and can provide assistance in predicting the degree of stability of atherosclerosis plaques.Early detection of LP-PLA2 level is conducive to the secondary prevention and individualized treatment of patients with atherosclerotic cerebral infarction,and can provide some clinical basis for the development of LP-PLA2 specific inhibitors.
