The Application Research Of Full-covered Self-expandable Metallic Stents And Uncovered Self-expandable Metallic Stents In The Malignant Biliary Obstruction

Objective To compare the clinical effect of Full-covered Self-Expandable Metallic Stents(FCSEMS)and Uncovered Self-Expandable Metallic Stents(USEMS)in biliary malignant obstruction.Methods The clinical data of patients with malignant biliary obstruction treated by Hepatobiliary Surgery in Hospital from January 2013 to June 2016 were retrospectively analyzed.All patients underwent B-,CT or MRI,MRCP and other images Check the scope of the expansion of the bile duct and obstruction site,combined with tumor markers to detect the cause of obstruction to make a clinical diagnosis.All patients underwent endoscopic implantation of FCSEMS or USEMS to tolerate obstruction.The patients were divided into two groups: FCSEMS and USEMSL.The effects of stenting,stenosis,survival time,stent shift rate,complication rate(pancreatitis,cholecystitis,cholangitis,perforation,Bleeding),obstruction causes.Results In this study,a total of 100 patients were collected,including 65 cases of USEMSL group,followed up 60 cases,35 cases of FCSEMS stent,follow-up 34,there was no significant difference between the follow-up rate(p = 0.312).There were no significant differences between the two groups(P <0.05).There were no statistically significant differences in gender,age,disease type and total bilirubin.There was no difference in the effect of lowering the levels of bilirubin(postoperative 3 days).There was no significant difference in the incidence of stent patency,short-term yellowing and the incidence of complications in patients with bare stents compared with bare stents.Conclusion The Full-covered Self-Expandable Metallic Stents and Uncovered Self-Expandable Metallic Stents in the help of Endoscopic retrograde cholangiopancreatography have the same clinical effect in releasing biliary obstruction and improving liver function.There is no significant difference in postoperative complications and stent patency and survival time.But Full-covered Self-Expandable Metallic Stents is easier to shift than Uncovered Self-Expandable Metallic Stents,The probability of recurrence of Full-covered Self-Expandable Metallic Stents was higher than that of Uncovered Self-Expandable Metallic Stents.
