Study On Investigation And Influential Factors Of Multi-point Practice Status Of Public Hospital Physicians

Objective As an important part of health care reform,multi-point practice is of great significance to the reform of China's medical and health system.The study draw questions from the doctors' and health managers' perspective,and take the doctors(intermediate title or above)in public hospitals in Ji'nan as the research object.Through the investigation of physician groups,understanding the current doctors' more practice to carry out the status quo in public hospitals of Ji'nan city,influencing factors and problems,and put forward to promote the practice of multi strategies to provide decision-making basis for health administrative departments,put forward suggestions for the development of multi physician practice.Methods Using literature retrieval method,retrieval related with the study of literature in How Net,Wan Fang database China;mastering the latest development of the relevant laws and regulations and related policies for the physician practice through access to the health and Family Planning Commission of Shandong Province,Ji'nan city health and Family Planning Commission official website and the government news network to prepare for relevant research.Questionnaires and interviews were conducted among selected physicians,administrators of some public hospitals and health administrative departments by using the questionnaire survey method,and multi stage stratified random sampling method,and free list interview method to understand the current situation,influencing factors and problems of multi physician practice.Statistical analysis was performed by using three line table,column list(chi square test)and logistic regression.Results and analysis1.Analysis on the current situation of multi physician practice Survey results show178 of the 360 physicians involved in the study were involved in more than one point of practice,or had multi-point practice behavior.Ways and means for doctors participating in the practice of multi-point practice is to implement the government directives(counterpart aid,counterpart aid),hospital cooperation under the medical conjoined mode,and there are fewer physicians applying for participation in multi-point practice to the health administration departments on their own initiative at the present stage.76.4% of physicians said they understood more about the practice of physician practices,and68.1% of the physicians indicated that there was an subjective idea to participate in more practice,and most physicians(91.1%)believe that they will choose to participate in more practice without hesitation.2.Analysis on the influencing factors of the willingness and behavior of physicians participating in multi-point practice The behavior of physicians participating in multi-point practice is driven by their motivation.The external influence factors of physician participation in multi-point practice intention and behavior are mainly manifested as the imperfect of macro policy and the supporting system,and physician units' factors and multiple practice units' factors etc.The internal influence factors of physician participation in multi-point practice intention and behavior are mainly expressed in the physician's own professional characteristics.Through the ?2test(alpha=0.05),it was found that there were significant differences in the willingness and behavior of physicians to participate in multiple practice.Finding that the Age,education and work experience are the influence factors of their participation in more practice will,and the gender,education,professional title are the influencing factors of the physician's participation in multi-point practice by using multiple logistic.Conclusion Physicians are more aware of the practice policy and have a strong desire to participate in many practices.At present,there are some problems such as slow development and unbalanced development in the practice of multi-point practice of doctors in Ji'nan;the motives of physicians participating in multi practice are mainly interest motivation and responsibility motivation;supporting system and legal framework is not perfect,it is urgent to solve the problem of multi physician practice.Therefore,the development of multi physician practice requires joint efforts of multi stakeholder groups.
