Objective:To investigate the distribution and properties of Denonvilliers fascia and peripheral autonomic nerves,and to provide a theoretical basis for the choice of surgical plane in radical resection of rectal cancer and the reduction of postoperative urination and reproductive dysfunction.Methods:Select complete and no malformed male pelvic specimens.The specimens were cut into the tissues containing the Denonvilliers fascia and its adjacent organs.The specimens were subdivided and then embedded in paraffin.Four consecutive slices were cut out from each tissue block,The first sections were stained with conventional HE and the remaining three were immunohistochemically stained with S100,Anti-Annexin V and anti-Agrin,respectively.Results:Denonvilliers fascia distributes the sensory nerves and motor nerves of the autonomic nerves,sensory nerves and motor nerves are independent of each other,and without the intercourse between each other,each of which is a single nature of the nerve.The pelvic plexus nerve fibers located on the lateral side of the Denonvilliers fascia are thicker,Denonvilliers fascia also has a few nerve fibers inside,constitute the traffic branch of the both sides of Pelvic plexus.The nerves located inside the Denonvilliers fascia also contain sensory nerves and motor nerves,and compared to its lateral nerve,the nerves inside the Denonvilliers fascia are more sparse.Although the distribution of nerves in the fascia of the Denonvilliers is more dispersed,the nerves behind the seminal vesicle have a tendency to gather.Conclusion:There are traffic branches of pelvic plexus In the Denonvilliers fascia,In the best way to preserve the patient's urinary and reproductive function,We need to choose different free planes according to the size and location of the tumor during radical resection of rectal cancer.A rectal tumor located in the anterior wall and invading or invaded the mesorectum,preferably in the front of the Denonvilliers fascia;For rectal tumors located on the sidewalls,or the tumor in the anterior wall but its stage is low and without penetrating the intestinal wall of the rectal,in order to achieve the purpose of protecting the nerve,the better free plane should be located behind the Denonvilliers fascia.On the determination of pelvic nerve properties,it helps to screen the factors that cause urinary incontinence,erectile dysfunction and ejaculation disorder in patients,which can contribute to deal with postoperative targeted therapy.
