| Objective:To investigate the correlation of serum cystatin C and vascular endothelial function in hypertensive patients with metabolic syndrome.Methods:Select 2015-01 to 2016-08 in shanxi medical university first affiliated hospital of hypertension of the merger of the metabolic syndrome patients with high blood pressure ward300(male 142,female 158)as the research object.The average age was 58.55 ±11.96 years old,the maximum was at the age of 75,and the minimum was at the age of 30.Selection with simple hypertension patients in hospital during the period of 150(male 77,female 73)in the control group,the average age was 56.96 ± 11.25 years old,the maximum was at the age of75,and the minimum was at the age of 30.Value for FMD ≥ 6%group and FMD < 6%group.The within MS group are divided into hypertension combined glucose abnormalitiesand abnormaland blood lipid group(group A,n = 72),high blood pressure combine blood glucose abnormalities and obesity group(group B,n = 70),hypertension combined dyslipidemia and obesity group(group C,n = 71),High blood pressure combine blood glucose abnormalities and dyslipidemia and obesity group(groupD,n = 87).The pure hypertension group according to the level of blood pressure are divided into high blood pressure level 1 group(group E,n = 45),high blood pressure level 2(group F,n = 48),high blood pressure level 3 group(group Gn = 57).Collectall the research object of general information,records of the patient’s age,gender,rise,weight,pulse,blood pressure,high blood pressure on admission to hospital medical history,smoking history,etc.Collecting serum cystatin C(Cysc),fasting blood glucose(FPG),serum triglyceride(TG),blood uricacid(UA),high density lipoprotein(HDL-C),serum low density lipoprotein(LDL-C)and other biochemical ultrasound index,brachial artery flow mediated vasodilation check Guide(FMD).To compare and analyze the general information of the two groups,and group factors and FMD to do single factor analysis and multivariate linear regression analysis.Results:1.(1)General data analysis showed that: in the two groups of patients,there was no statistically significant difference in gender ratio and proportion of smokers.The group With metabolic syndrome(MS)serum total cholesterol(TC),triglyceride(TG),blood glucose,body mass index(BMI),serum uric acid,serum homocysteine(HCY),cystatin C,urine(urine albumin MA)was higher than the control group(P < 0.05),but the serum high density lipoprotein(HDL-C)and FMD are lower than that of the control group(P < 0.05),other indexes had no significant difference between the two groups.(2)Combined with MS group compared with FMD group≥6%,FMD < 6% group serum cystatin C level is lower(P < 0.05),and hypertension group compared with FMD group≥6%,FMD < 6% group serum cystatin C levels had no significant difference(P>0.05).(3)Pure hypertension group E,F,G between FMD and urinary inhibition C levels of serum comparison shows that FMD value with elevated blood pressure levels,FMD value showed a trend of decrease(E > F >G,P <0.05),serum(inhibition difference of content of C,no statistical significance(P > 0.05).(4)According to the serum level of cystatin C combined MS group were divided into less than0.8(n = 100),(0.8~0.95]((n = 39),(0.95~1.11](n = 66),≥ 1.11(n = 95),FMD levels of 4groups respectively(6.83 ± 2.64)% 、(6.02 ± 2.11)%、(5.37 ± 1.65)% 、(4.29 ± 2.16)%,with the increase of serum cystatin C level,FMD showed a decreasing trend.2.(1)The correlation analysis showed that FMD and merge in MS group age,blood glucose,TG,uric acid,serum creatinine,blood serum,cystatin C were nagative correlation,was positively related with HLD-C(r=-0,258、-0、156,-0.147、-0.173、-0.178、-0.279、0.143,P < 0.05),with a body mass index(BMI),syetolic pressure,diastolic blood pressure,heart rate,total cholesterol,low-density lipoprotein cholesterol(hdl-c),serum homocysteine(HCY),hypersensitive c-reactive protein,urine trace albumin has no statistically significant difference(P > 0.05).FMD and negatively correlated with simple hypertension group age isnegatively related to and the HLD-C is positively correlated(P < 0.05).(2)In the MS group,A,B,C,D in single factor correlation analysis of serum cystatin C and FMD showed that with the increase of serum cystatin C level,FMD decreased(r =-0.245,-0243,-0.272,-0.342,< 0.05).3.Multiple linear stepwise regression analysis showed that the FMD% as the dependent variable,age,blood glucose,TG,serum uric acid,serum creatinine,serum cystatatin C,high density lipoprotein as independent variable stepwise multiple linear regression analysis.The results showed that the metabolic syndrome group age,Cystatin C,blood glucose,triglyceride(TG)is FMD effect,and simple hypertension group is only age and influencing factors of FMD.Conclusion:It was Negative correlation of serum cystatin C level and vascular endothelial function in hypertensive patients with metabolic
