| Objective:To evaluate endoscopic ultrasonography(EUS)differential diagnostic value of eminence lesions of upper gastrointestinal tract.Methods:A retrospective analysis was made based on clinical and pathological materials of 98 upper gastrointestinal eminence lesions patients from July 2010 to August 2016 in The First Affiliated Hospital of Shanxi Medical University.EUS findings were compared with postoperative or endoscope treatment or biopsy pathological diagnosis,and the sensitivity,specificity and accuracy of EUS differential diagnosis in property and layer origin of upper gastrointestinal eminence lesions were calculated.Results:Retrospectively collected 98 lesions in 39 cases of esophagus,8 cases of Cardia,45 cases in the stomach,6 cases of duodenal,48 male,female 50,Minimum age 32 years old,biggest 81 years old.The lesions originate from the mucosa in 39 cases,33 cases of mucosa layer.submucosa in 11 cases,muscularis in 15 cases.The size of these lesions was7.04±4.82 mm,the minimum value is 1.2 mm,the maximum is 26.2 mm.Acquire the method of pathological biopsy,endoscopic treatment,surgery.Tissue biopsy 7 cases(including polyps in 4 cases,3 cases of early carcinoma),treatment of 59 cases of EMR,ESD treatment of 29 cases;The other 3 cases(both for stromal tumor)with intraoperative bleeding or perforation underwent total laparoscopic assisted layer.The sensitivity,specificity and accuracy of EUS differential diagnosis in upper gastrointestinal eminence lesions property were 89.80%,96.58%,96.16%.The sensitivity,specificity and accuracy of EUS differential diagnosis in layer origin of upper gastrointestinal eminence lesions were 97.96%,99.05%,98.44%.Conclusion:EUS has value in the differential diagnosis of property of upper gastrointestinal eminence lesions,and has high accuracy in identifying the origin layer,which is helpful to choose therapeutic schedule of upper gastrointestinal tract eminence lesions under endoscopy. |
