Clinical Research Of Endoscopic Ultrasonography In Diagnosing And Minimally Invasive Treatment In Eminence Lesions Of Upper Digestive Tract

Objective:The aim of this study was to investigate whether EUS is of assistance in distinguishing eminence lesions of upper digestive tract, and to aquire the measures and clinical value of minimally invasive treatment in patients with eminence lesions of upper digestive tract.Methods:The first section:The clinical records of194patients with eminence lesions of upper digestive tract from January2009to January2011was reviewed retrospectively with the examination of BUS、CT、EGD、UGC and EUS. All patients, including101men and83women with a mean age of58.7years. We evaluated the advantage and disadvantage of each examination, and decided which was the best method in diagnosis eminence lesions of upper digestive tract.The second section:There were126patients with eminence lesions of upper digestive tract, of which61were received endoseopic treatment,65received laparoscope treatment. we observed each methods, evaluated the clinical effection of minimally invasive treatment.Results:Among the194patients,167final diagnoses were confirmed by histopathologically、,27extended clinical follow-up.Sensitivity, specificity, positive predictive values, negative predictive values, accuracy of EUS for eminence lesions of upper digestive tract were94.4%、89.5%、83.7%、91.1%、89.3%.Endoscopic sueeess rate was78.4%in patients with eminence lesions of upper digestive tract. CT sueeess rate was62.8%in patients with eminence lesions of upper digestive tract. UGC sueeess rate was58.8%in patients with eminence lesions of upper digestive tract. In126patients, EUS scanning in the mucosa, submucosa and muscularis mucosa of the upper gastrointestinal tract of elevated lesion in61patients, one line of successful endoscopic treatment for57patients, treatment success rate was93.4%,3cases had complications, the complication rate was4.9%; EUS scanning in the muscularis propria layer stomach lesions65cases, in which laparoscopic treatment for62cases of successful treatment,95.4%success rate, complications of4cases, the complication rate was6.2%.Conclusion:EUS was the best method in diagnosis of eminence lesions of upper digestive tract of the study,.Beeause of minimally-invasive, Endoseopy and Laparoscope in patients with eminence lesions of upper digestive tract was less invasive、safe、 effective.
