Objective:1.The research of musculoskeletal ultrasound interventional injection therapy combined with rehabilitation training on shoulder hand syndrome after stroke in patients with clinical curative effect,at the sametime in comparsion with physical factors(the microwave treatment group).2.To further explore causes of shoulder hand syndrome.Method:42 stroke patients with shoulder hand syndrome stage from January 2014 to July2016 in our hospital were divided into microwave combined with routine rehabilitation training group(19 cases)and interventional ultrasound combined with routine rehabilitation training group(23 cases).The treatment group was given hemiplegic shoulder joint soft tissue around the musculoskeletal ultrasound guided injection Compound Betamethasone Injection treatment on the basis of routine rehabilitation therapy,the control group on the basis of routine rehabilitation therapy of hemiplegic shoulder joint to microwave treatment.By comparing before and after treatment,respectively,before treatment,after treatment and 3 10 d using visual analogue scale(VAS)and the corresponding assessment of shoulder The degree of joint pain,hand and metacarpophalangeal joint small edema,respectively before and after the treatment,10 d was used to evaluate the motor function of upper limbs simplified Fugl-Meyer scale(FMA)and Barthel index(BI)of upper limb motor function and activities of daily living.Result:Therapeutic ultrasound interventional therapy group and microwave group two groups of patients before the VAS,FMA,BI,the back of hand and palm refers to small joint swelling of the score,there was no obvious statistical significance difference(p > 0.05).2 groups of patients with edema score,VAS score after treatment with the change of time all is on the decline,especially in 3 days after treatment edema VAS score,score the ultrasound interventional therapy group has significantly lower speed;Slow down and small amplitude in physical therapy group.ultrasound interventional therapy group after treatment for 3 days,10 days after treatment,the VAS oedema circumstance,hand and hand refers to the joints between the score of each point in the microwave group were significantly lower,compared the difference had statistical significance(P < 0.05).The scores of FMA and BI in the two groups increased with time after treatment,but there was no significant difference between the treatment group and the control group in the FMA and BI scores after 10 days(P > 0.05)Conclusion:1.Ultrasonic interventional injection has obvious effect on improving the shoulder pain and the edema of the dorsum of hand and metacarpophalangeal joints in patients with shoulder hand syndrome after stroke,and the effect of ultrasonic interventional group is better than that of microwave group.2.Ultrasound interventional therapy can improve shoulder hand syndrome after stroke in patients with shoulder pain,hand and hand refers to the small joint swellingin a short time,rehabilitation treatment for early,timely intervention,to further promote side upper limb functional recovery,improve the ability of daily life,improve the patients quality of life.3.The incidence of shoulder hand syndrome is related to rotator cuff injury,shoulder joint cavity effusion,and tendon sheath of the biceps brachii muscle(especially the subacrmial-subdeltiod bursa effusion).
