ObjectiveTo study the influence of DMT1 gene polymorphism on lead poisoning susceptibility in battery enterprise workers.Methods1.812 workers in 2 battery companies were adopted in cluster sampling.Baseline characteristics,occupational exposures and the information of occupational physical examination of each worker were recorded in detail.Unconditional logistics regression model was performed to analyze the related factors of high blood lead.2.DNA was extracted from the blood of 735 workers and 3 SNPs in DMT1 were detected by the technical platform of Sequenom.Unconditional logistics regression was performed to analyze the effect of DMT1 gene polymorphism on high blood lead in different genetic patterns and the interaction between environment and genetics.3.We constructed the LASSO model to screen the influencing factors of high blood lead.Receiver-operating characteristic(ROC)curve analysis were also performed.Results1.The rate of lead smoke and lead dust in the workplace air was 43.39% and the proportion of high blood lead(blood lead≥400μg/L)in the study subjects was 30.07%.We found the gender,education level,length of service,lead exposure and alcohol consumption were associated with risk of high blood lead(the ORs were 0.311(95% CI: 0.186-0.519)for females,0.361(95%CI:0.246-0.531)for the education level in junior high school and above,2.023(95%CI:1.385-2.954)for working years,3.802(95%CI:2.613-5.532)for high exposure of lead and 1.750(95%CI:1.030-2.972)for Drink more than 4 days a week).High blood lead may be a risk factor for hypertension(the OR was 1.657(95%CI:1.052-2.608)).The effect of blood lead on hemoglobin,white blood cell,urinary protein and urinary occult blood were not statistically significant(P> 0.05).2.The genotype of rs149411 and rs224446 can increase the risk of high blood leadunder dominant genetic model,the ORs were 1.423(95%CI: 1.005-2.015)and 1.539(95%CI:1.040-2.277),respectively.rs149411 carrying CC/TC genotype combined with high lead exposure can increase the risk of high blood lead(OR=4.959,95%CI: 1.040-2.277),this site has an additive interaction with lead exposure levels RERI=2.255(95%CI:0.245,4.266),AP=0.455(95%CI:0.161-0.748),S=2.324(95%CI: 1.004-5.380).rs224446 carrying with GG/GA genotype combined with lead exposure or alcohol drinking can increase the risk of high blood lead(OR=4.876,95%CI: 2.788-8.525).We were not found that this site had an additive interaction with lead exposure and alcohol drinking,and the multiplication interaction with lead exposure and alcohol consumption in both the rs149411,rs224446 were also not been found(P> 0.05).3.There were 13 variables associated with high blood lead selected by the lambda.min prediction model,such as gender,length of service,education level,lead exposure level,rs149411 dominant genetic model,rs224446 dominant genetic model and so on.The ability of lambda.min model was superior to lambda.1se model(area under the curve(AUC)0.792 vs 0.772,respectively;Z=2.816,p<0.05).The sensitivity,specificity,the positive likelihood ratio,the negative likelihood ratio,the positive predictive value,the negative predictive value and the accuracy rate of the model were 0.765,0.698,2.54,0.34,0.523,0.873 and 0.718 respectively.Conclusions1.Gender,length of service,education level,lead exposure level and drinking frequency were associated with high blood lead in the battery workers.2.The polymorphism of rs149411 and rs224446 were associated with high blood lead.There was additive interaction between rs149411 polymorphism and lead exposure.3.Lambda.min was the best predictor.There were 13 variables associated with blood lead levels:gender,length of service,education level,lead exposure level,rs149411 dominant genetic model,rs224446 dominant genetic model and so on.
