Objective:Analysis the clinical feature of neuromyelitis optica spectrum disorder related optic neuritis,to investigate the optic neuritis prognosis of patients with NMOSD who was either AQP-4 seropositive or MOG seropositive or/and LETM.providing a theoretical basis for clinical treatment.Methods:All data of patients with ON or NMO or NMOSD related optic neuritis were retrospectively collected in our ophthalmology and neurology department from 2013-01-01 to 2016-11-30.Those patients were divided into six groups as AQP4-Ab seropositive group?AQP4-Ab seronegative group?MOG-Ab seropositive group?MOG-Ab seronegative group?and not tested group?MRI?3 LETM group.Results: In 196 cases,male to female ratio is 1:3.Idiopathic demyelinating optic neuritis multiple sclerosis related optic neuritis accounted for 25%,NMOSD accounted for 12%,the other cannot be classified accounted for 38%.Neurology Department of optic nerve involvement NMOSD account 60%.54 patients were diagnosed,40 cases of optic neuritis were tested for AQP-4 MOG,MBP antibody?Comparing AQP4-Ig G(+)group and AQP4-Ig G(-)group,MOG-Ig G(+)group and MOG-Ig G(-)group and non tested group there were not significant differences between the five groups(P>0.05);the maintain visual acuity between the five groups were statistically differences significant among those groups(P < 0.05);comparing AQP4-Ig G(+)and AQP4-Ig G(-)group,AQP4-Ig G(+)and MOG-Ig G(+)group,AQP4-Ig G(-)and MOG-Ig G(-)group,AQP4-Ig G(-)and non tested group,MOG-Ig G(+)and non tested group was statistically differences significant between those two groups(P < 0.05);comparison two groups as below: AQP4-Ig G(+)and MOG-Ig G(-)group,AQP4-Ig G(+)and non tested group,AQP4-Ig G,(-)and MOG-Ig G(+)group,MOG-Ig G(+)and MOG-Ig G(-)group,MOG-Ig G(-)group and non tested group,there is no Significance difference between all of them(P > 0.05).Compared with LETM group,AQP4-Ig G(+),MOG-Ig G(+)there was no significant difference in the incidence of vision between the three groups(P > 0.05);compared with LETM group,AQP4-Ig G(+).MOG-Ig G(+)group to maintain visual acuity in outcome between the three groups was statistically differences significant(P=0.007,P < 0.05);group LETM and AQP4-Ig G(+)patients to maintain visual acuity was no statistically significant difference(P > 0.05);group LETM and MOG-Ig G(+)to maintain visual acuity was statistically significant group differences in prognosis(P < 0.05).Magnetic Resonance Image exists long segment longitudinal extension of the transverse myelitis(Longitudinally extensive transverse myelitis,LETM)showed recurrence or permanent vision damaged.In Ophthalmology about Visual evoked cortical potentials tested about 88.9% of patients P100 wave prolong and amplitude go down.During long-term follow-up,AQP4-Ig G(+)expression of optic neuritis cases recurred 2 times on average,longitudinally extensive transverse myelitis lead to permanent vision loss.Conclusion: Neuromyelitis optica spectrum disorder associated optic neuritis longitudinally extensive transverse myelitis and AQP4-Ig G seropositive lead to worse Visual acuity prognosis,MOG-Ig G seropositive prognosis was much better;The expression of MOG antibody may be a new optic nerve subtype in optic neuritis,as initial or recurrent optic neuritis early diagnosis is very important.
