Background and ObjectiveThe treatment and prognosis of primary esophageal small cell carcinoma(PESCC),an uncommon esophageal malignant tumor,have seldom been reported.To analyze clinical data and prognostic factors in the patients with esophageal small cell carcinoma,and to further explore its treatment modality,so as to provide theoretical basis for evaluation of prognosis and choice of reasonable treatment modality.MethodsWe retrospectively analyzed the clinical data of 41 Patients with primary esophageal small cell carcinoma proved by gastroscope/esophagoscopy or operation pathologically had been selected from the Fujian Medical University Union Hospital between June 2006 andNovember 2015.The 41 patients with small cell carcinoma of the esophagus surgery plus chemotherapy in 13 cases,19 cases pure radiation and chemotherapy,8 cases of simple surgery,Another 1 case patients refused to accept the treatment.The number of cycles of chemotherapy was <4 in 8 cases,?4 cycle in 11 cases.Full set of 41 patients.There are 39 cases received followup.The curative effect of 2 cases lost visitor.Survival conditions were analyzed,and the effects of pathological stage and treatment modality on survival condition were explored.Besides,prognostic factors were analyzed.Results1.41 cases of patients were treated with gastroscopy/esophagoscope,and only 21 were diagnosed as small cell carcinoma.The diagnosis rate was 51.2%.2.There are 31 males and 10 females in 41 cases;Aged 43-71(median age 59)years old,beyond 60 years old acounts 18 cases(43.9%);smoking,drinking history in 27 cases(65.9%);Tumors located in the middle thoracic for the highest proportion,accounting for 24 cases(58.54%),The upper thoracic portion 7 cases,and 10 cases of lower thoracic segment;lesions,1-8 cm in length,average of 3.61 cm,where < 5 cm of 27 cases(65.85%),5 cm or higher for 14 cases(34.15%);Patients with stage I of 6 cases(14.63%),accounting for 10 patients with stage II(24.39%),patients with stage III accounted for 11 cases(26.83%),14 cases(34.15%)patients with stage IV.Among them,the length of lesion and the prognosis of tumor staging in patients with PESCC has significant effects(P < 0.05).3.A total of 39 cases received follow-up,the overall median survival was 20.8 months,1,2,3 year survival rate were 68%,37% and 20%,respectively.Tumor length < 5 cm patients 1,2,3 survival rate and the median survival time were higher than the tumor patients with a length of 5 cm or more(81%,48%,30%,24.75 mo vs 43%,14%,0%,12 mo);Tumor staging for phase I and II patients 1,2,3 year survival rate and the median surial high phase III and IV patients(23 mo vs 94%,38%,31%,94%,36%,12%,15.5 mo).4.Comprehensive treatment in patients with relatively simple chemotherapy patients survival rate increased significantly(p < 0.05).For chemotherapy patients,the survival rate is more than 4 cycle chemotherapy patients was significantly higher than that of < 4 cycle chemotherapy patients(p < 0.05).Conclusions1.Esophagus small cell carcinoma by microscopy diagnosis was relatively low.2.The survival rates at stage I and stage II have advantages than that at stage III and stage III,but the survival rates between the two stages show no significant difference.3.The survival rate of patients with surgery combined with chemotherapy was higher than that of chemotherapy alone.There is no significant difference in survival rate between surgery combined with chemotherapy and simple surgery.4.Patients with chemotherapy cycle ? 4 cycles have a better survival rate than that with < 4 cycle.5.Primary esophageal small cell carcinoma is a malignant tumor with early metastasis and poor prognosis.Surgery combined with chemotherapy is a main therapeutic method for primary esophageal small cell carcinoma.It is helpful to extend the survival time of patients.
