Survival Analysis Of Patients With Oral Squamous Cell Carcinoma Treated By Radical Surgery For The First Time

Objective: To investigate the five-year overall survival(OS)rate and postoperative survival time of patients with oral squamous cell carcinoma(OSCC),and to analyze the clinical and pathological factors affecting the survival of patients with OSCC.Methods: To collect the inpatient data of patients with OSCC admitted to the department of maxillofacial surgery of the first affiliated hospital of chongqing medical university from 2011.4 to 2013.12,and a total of 78 patients who received radical surgical treatment for the first time were obtained.Follow-up was conducted through outpatient follow-up,telephone follow-up and electronic medical record tracking until March2019.To retrospectively reviewed the clinical,pathological and follow-up records.Descriptive analysis was conducted on counting and measurement data;survival was calculated using the Kaplan-Meier method and differences in OS rate were analyzed by log-rank test;prognostic factors were using the univariate and multivariate analysis Cox proportional hazard model.Differences with a P value less than 0.05 were consideredstatistically significant.Results: A total of 68 patients were included in this study,with a male/female ratio of about 2:1,an average age of 60.9±10.6 years,and a median course of disease of 5(3-12)months.Tongue(35.3%)was the most common part,followed by cheek(30.9%).The 5-year OS rate was 55.9%,about 52.9% of OSCC patients had recurrence and metastasis after surgery,and the 5-year overall disease-free survival rate was 42.6%.In the 30 death cases who died of this disease within 5 years after surgery,the shortest survival time was 6 months,the longest survival time was 52 months,and the median survival time was 20.5(10-40)months.The univariate analysis showed that clinical stage,size of tumor,lymph node metastasis(pN),pathological differentiation and recurrence and metastasis were the exposure factors affecting survival time(P < 0.05).Multivariate analysis showed that pathological differentiation and recurrence and metastasis were independent factors affecting survival time(P < 0.05).Another special finding was that 4 patients with oral squamous cell carcinoma(OSCC)were found to suffer from esophageal squamous cell carcinoma(ESCC)within 1-4 years after surgery,and the proportion of patients with OSCC combined with ESCC accounted for 5.1%.Among them,3 patients died of ESCC within 1-2 years after oral surgery without any symptoms of recurrence and metastasis of OSCC.Conclusion: The survival prognosis of patients with oral squamouscell carcinoma is correlated with clinical stage,size of tumor,lymph node metastasis(pN),pathological differentiation,recurrence and metastasis,and pathological differentiation and recurrence and metastasis were independent factors affecting survival time,and low differentiation and the occurrence of recurrence and metastasis indicate worse survival.Some OSCC patients may be screened for esophageal cancer.Clinical screening of esophageal squamous cell carcinoma may be performed routinely in OSCC patients with a history of alcohol and tobacco.
