Exploratory Research Of Contrast-enhanced Ultrasound In Distinguishing Polypoid Lesions Of The Gallbladder

Objectives: To explore the differentiating ability of contrast-enhanced ultrasound(CEUS)in polypoid lesions of the gallbladder(PLG)via comparing the perfusion features of different lesions during this examination.Methods: A total of 40 patients with PLG from September 2014 to March 2017 in our hospital were selected as the subjects,13 male and 27 female included.According to the pathological results,the patients were divided into benign group and malignant group.There were 33 cases in benign group,including cholesterol polyps(CP)in 19 cases,adenomyomatosis in 7 cases,chronic cholecystitis(CC)in 4 cases,and gallbladder adenoma(GA)in 3 cases,and malignant group included gallbladder carcinoma(GC)in 7 cases.The results were reviewed by senior ultrasound doctor,using acoustic quantitative analysis software to analyze the video.The result included size,number,echogenicity,wall integrity and vessel signal within two-dimensional ultrasound and arrival time,peak time,wash out time,distribution,degree,wall integrity and vessel signal within CEUS.The data in each group were analyzed by statistical software to compare the difference among PLG in CEUS.Results: Comparisons between benign group and malignant group are as follows.The arrival time was 19.76 ± 0.67(s)and 15.00 ± 1.45(s).The peak time was 30.79 ± 1.06(s)and 23.57 ± 1.57(s).The wash out time was 51.42 ± 1.77(s)and 30.14 ± 1.71(s).There were 26 homogeneous cases and 7 heterogeneous cases in benign group,the result in malignant group was 2 and 5.There were 4 hyper enhancement cases in benign group and the other 29 cases were not,the result in malignant group was 4 and 3.There were 4 wall-interrupted cases in benign group and the other 29 cases were not,the result in malignant group was 6 and 1.There were 29 dotted vessel cases in benign group and the other 4 cases were not,the result in malignant group was 2 and 5.The difference was all significant(P<0.05).Comparison between CP and GA is as follows.The arrival time was 19.84 ± 0.64(s)and 15.67 ± 4.06(s).The peak time was 31.26 ± 1.08(s)and 22.33 ± 5.21(s)The wash out time was 48.53 ± 1.42(s)and 41.00 ± 1.00(s).There were 16 homogeneous cases and 3 heterogeneous cases in CP,the result in GA was 3 and 0.There were 3 hyper enhancement cases in CP and the other 16 cases were not,the result in GA was 1 and 2.There were 3 wall-interrupted cases in CP and the other 16 cases were not,the result in GA was 0 and 3.There were 15 dotted vessel cases in CP and the other 4 cases were not,the result in GA was 3 and 0.There was significant difference only in peak and wash out time(P<0.05).Comparison between adenomyomatosis and GC is as follows.The arrival time was 19.29 ± 1.30(s)and 15.00 ± 1.45(s).The peak time was 30.57 ± 2.38(s)and 23.57 ± 1.57(s)The wash out time was 63.57 ± 5.05(s)and 30.14 ± 1.71(s).There were 3 homogeneous cases and 4 heterogeneous cases in adenomyomatosis,the result in GC was 2 and 5.There were 0 hyper enhancement cases in adenomyomatosis and the other 7 cases were not,the result in GC was 4 and 3.There were 1 wall-interrupted cases in adenomyomatosis and the other 6 cases were not,the result in GC was 6 and 1.There were 7 dotted vessel cases in adenomyomatosis and the other 0 cases were not,the result in GC was 2 and 5.There was no significant difference only in distribution and degree(P>0.05).Conclusions: The arrival peak and wash out time in malignant PLG were earlier than those in benign PLG.The malignant PLG was more likely to act as heterogeneous hyper enhancement wall-interrupted and non-dotted vessel pattern,and benign PLG was just the opposite.The peak and wash out time in GA were earlier than those in CP.The other comparison between GA and CP didn't show any significant difference.The arrival peak and wash out time in GC were earlier than those in adenomyomatosis.The GC was more likely to act as wall-interrupted and non-dotted vessel pattern,and adenomyomatosis was just the opposite.The other comparison between GC and adenomyomatosis was just the opposite.
