| Objective:By comparison the Bladder Tumor Associated Antigen Quantitation Detection Kit(Chemiluminescent Immunoassay)and urine exfoliative cytology in the diagnosis of bladder cancer,and analysis of two methods for the different characteristics of the diagnosis of bladder cancer,found to affect the detection of factors,trying to find a simple,cheap,efficient means for the diagnosis of bladder cancer and recurrence of bladder cancer.Methods:In this study,a total of 121 patients with suspected bladder cancer.All the patients were examined with bladder tumor antigen and three times urine exfoliative cytology before operation.Detection of bladder tumor antigen by the Bladder Tumor Associated Antigen Quantitation Detection Kit,which be invented by Beijing PerGrande Biotechnology Development Co.Ltd.;urine cytology were collected in three consecutive days of urine inspection,inspection by the Department of pathology General Hospital Affiliated to Tianjin Medical University professional pathology physicians completed,if one finds the bladder tumor cells were determined as positive.All patients after surgery to determine the pathological results,failure to surgery or can not get the results of pathology patients out of the group.The results of analysis,the first confirmed by the postoperative pathological results,quantitative analysis of selected the Bladder Tumor Associated Antigen Quantitation Detection Kit(Chemiluminescent Immunoassay)using ROC curve of the cut-off value,the measured value is less than the value determined as negative,greater than or equal to this value is judged to be positive.The sensitivity and specificity of the two methods were analyzed,and the ROC curves of the two methods were calculated,and the diagnostic values of the two methods were compared.By the postoperative pathological results,we can also be for patients with bladder tumor staging and grading,and calculation of different stages,the sensitivity of the detection means,the specific classification of two species,watch two kinds of diagnostic value of detection method on different stage and grade of bladder tumor.Result:In this study,30 patients with no pathological results were excluded,and a total of 91 patients were enrolled in the study.Bladder cancer group 34 case,the patients with pathological high grade tumors accounted for 23 cases,low grade tumors accounted for 11 cases;according to the tumor stage,stage T1 accounted for 10 cases,T2 accounted for 22 cases,more than T3 accounted for 2 of cases.Non bladder tumor group 57 cases.Through statistical software SPSS,we selected <84 ng/ml as normal reference value,this standard,bladder tumors group: 26 cases were judged as positive,8 cases were judged as negative;non bladder tumor group: 23 cases were judged as positive,34 cases were judged as negative.The sensitivity of bladder tumor associated antigen quantitative detection kit to bladder cancer was 76.5% and specificity was 59.6%.SPSS software was used to determine the ROC curve of bladder cancer associated antigen quantitative determination kit.The area under ROC curve of AUC was 0.681.The sensitivity of urine cytology was 61.8% and the specificity was about 91.2%.The 3 results of urine exfoliative cytology were calculated separately,and the sensitivity of urinary cytology was 22.5% and the specificity was 91.8%.SPSS software was used to determine the ROC curve of bladder cancer associated antigen quantitative determination kit.The area under ROC curve of AUC was 0.765.In this study,the patients were divided into T1 group and T2-T4 group according to the pathological stage.The patients were divided into low grade group and advanced group according to the pathological grade.There were 10 cases in the T1 group,the sensitivity of the tumor associated antigen quantitative determination kit was 40%,while the sensitivity of the urine exfoliative cytology was 10%.The T2-T4 group has a total of 24 cases of bladder tumor associated antigen,the sensitivity of the quantitative assay kit was 91.7%;and the sensitivity of the determination of urine cytology was was 83.3%.The low level group there were 11 cases of bladder tumor associated antigen,the sensitivity of the quantitative assay kit was 45.5%;and the sensitivity of the determination of urine cytology was was 18.2%.The high level group there were 23 cases of bladder tumor associated antigen,the sensitivity of the quantitative assay kit was 91.3%;and the sensitivity of the determination of urine cytology was was 82.6%.Conclusions:1 The diagnostic value of the Bladder Tumor Associated Antigen Quantitation Detection Kit and urine exfoliative cytology test is not significant,and both of them can be used as an auxiliary means of cystoscopy;2 Bladder Tumor Associated Antigen Quantitation Detection Kit for bladder tumor has a certain diagnostic ability,compared with the urine exfoliative cytology,high sensitivity,specificity is not as good as urine cytology;3 Bladder Tumor Associated Antigen Quantitation Detection Kit and urine exfoliative cytology in diagnosis of high grade and high grade tumors has more advantages compared with low stage and low grade tumors;4 The results of the quantitative determination of bladder tumor associated antigen were significantly affected by gross hematuria. |
