| Objective:To evaluate the feasibility that infrared thermal imaging technology used in analysis on the curative effect of interventional therapy for Cervicalspondylotic radiculopathy caused by Cervical disc herniation,objective to find a method to objectively evaluate the herniation of cervical disc;.To observe the clinical efficacy of C guided radiofrequency ablation combined with collagenase lysis and ozone.Methods:Collecting 50 patients diagnosed Cervicalspondylotic radiculopathy caused by Cervical disc herniation in pain department of Tianjin First Center Hospital form December 2014 to January 2016.All patients were in good physical condition and did not undergo open surgery before interventional therapy.To inform patients about the risk of interventional therapy.After entering the room,the patient took the dorsal decubitus,connected the monitor to monitor the patient’s vital signs,and established the venous access.Under the guidance of "C" arm X-ray,the cervical intervertebral disc puncture was performed by the right side of the cervical spine.After successful puncture,cervical disc plasma radiofrequency ablation and ozone injection were performed in the cervical spine.Then lie on the position on uninjured side,C arm localization,cervical intervertebral foramen puncture.After the puncture were given 1%lidocaine 1ml,10 minutes after the monitoring without spinal anesthesiawere respectively injected Diprospan 3mg and collagenase 800 units.Keep healthy position 6 h after the surgery,One day before surgery(T0)and 6months after operation(T1),the clinical evaluation scale(CASCS)score and the infrared thermal imaging of neck and shoulder were collected and compared.Results:1 Changes of CASCS score in patients at T0 and T1The CASCS score of patients was 79.12±2.73 at T0,The CASCS score of patients was 88.30±2.66 at T0.The CASCS score of T1 was significantly higher than that score of T0 score.The difference of CASCS score T0-T1 called d1 is-9.18±2.79.The difference of CASCS score between T0 and T1 was statistically significant(P < 0.01).2 Changes of temperature difference of bilateral neck and shoulder in patients at T0 and T12.1 Temperature of bilateral neck and shoulder temperature difference in patients at T0The healthy side temperature value of patients at T0 was(30.28±0.45)℃,The affected side temperature at T0 was(31.58±0.52)℃.The D-value ΔT0 is(1.33±0.34)℃.There was a significant difference in T0 between the healthy side and the affected side(P < 0.01).2.2 Temperature of bilateral neck and shoulder temperature difference in patients at T1The healthy side temperature value of patients at T1 was(29.95±0.53)℃,The affected side temperature at T1 was(30.31±0.43)℃.The D-value ΔT1 is(0.34±0.23)℃.There was a significant difference in T1 between the healthy side and the affected side(P < 0.05).2.3 Comparison ofΔT0 andΔT1ΔT0-ΔT1 was d2(0.94±0.34)℃,There was significant difference betweenΔT0 andΔT1(P < 0.01).3 Correlation analysis between CASCS score and infrared thermal imaging of neck and shoulder temperatureThe correlation analysis between d1 and d2 showed that the difference of CASCS score was negatively correlated with the temperature difference between the healthy side and the neck and shoulder.Conclusion:Radiofrequency ablation combined with collagenase lysis and ozone has obvious curative effect on Cervicalspondylotic radiculopathy caused by Cervical disc herniation,Infrared thermal imaging technology can objectively evaluate the condition and curative effect of patients with cervical disc herniation. |
