Objective:1 To evaluate auditory and speech development of patients under 6 years with bilateral conductive hearing loss after using Brand Sophono softband type of bone conductive hearing aids;2 To investigate hearing improvement of patients above 6 years who suffered bilateral conductive hearing loss after using the Sophono softband;3 To understand hearing and speech improvement of patients above 6years with single side deafness.Methods:From July 2015 to April 2016,27 patients met the inclusion criteria,which were assigned into three groups.Preliminary analysis of auditory and speech improvement of patients and evaluation of clinical application efficacy of three groups of patients after using Sophono softband.1 Inclusion criteria: 1)Patients with conductive or mixed hearing loss:age? 1,bone conductive thresholds ? 30 dB HL,air-bone gap > 30 dB;2)SSD(single side deafness): age?6,air conductive PTA of injured ear?90 dB HL,bone conductive PTA of normal ear?20 d B HL,patients with no vertigo,tinnitus and other symptoms except unilateral profound sensorineural hearing loss.2 Experimental groups: 1)under 6-year-old with bilateral conductive hearing loss group(n=11,aged at a range of 1 to 5 years,an average of2.64±1.03 years);2)above 6-year-old with bilateral conductive hearing loss group(n=12,aged at a range of 9 to 24 years,an average of 13.58±5.44 years);3)above 6-year-old with unilateral profound sensorineural hearing loss group(n=4,aged at a range of 14 to 67 years,an average of 39.75±21.79years).3 Audiological evaluation: 1)The bone auditory thresholds of the under6-year-old with bilateral conductive hearing loss group were assessed by auditory brainstem response(ABR).Soundfield pure tone threshold average(PTA)was applied unaided and aided for the day,2 weeks and 3months;Categories of auditory performance(CAP)was conducted to evaluate the speech development of under 6-year-old with bilateral conductive hearing loss group aided 2 weeks and 3 months;2)Soundfield PTA of above6-year-old group was applied unaided and aided for the day,2 weeks and 3months;Speech recognition threshold(SRT)were conducted by Chinese open speech evaluation system of angel training.To record adverse reactions and adverse events of all the patients at the same time.Results:1 For the under 6-year-old with bilateral conductive hearing loss group,the bone auditory thresholds ABR even was 14.09±5.84 dB nHL.The soundfield PTA even was 75.68±13.64 dB HL unaided versus 36.59±9.88,35.00±5.92,33.40±4.22 dB HL aided for the day,2 weeks and 3 months.The differences of soundfield PTA even between unaided and aided for the day,2weeks and 3 months was significant(all P<0.05),but the aided for the day,2weeks and 3 months was no significant between each other(all P>0.05).The auditory thresholds of 0.25,0.5,1,2,4 kHz aided were declined 31.82 ±12.90,35.00±13.78,48.19±14.19,41.81±12.10,44.09±17.86 dB respectively.The CAP score even improved to 5.36±0.50,5.36±0.50 aided for 2 weeks and3 months versus 2.64±0.50 unaided;2 For the above 6-year-old with bilateral conductive hearing loss group,the bone auditory thresholds PTA even was 7.00±6.67 d B HL.The soundfield PTA even was 60.52±9.13 dB HL unaided versus 26.35 ± 5.55,29.38±4.69,31.35±9.83 dB HL aided for the day,2 weeks and 3 months.The differences of soundfield PTA even between unaided and aided for the day,2 weeks and 3months was significant(all P<0.05),but the aided for the day,2 weeks and 3months was no significant between each other(all P>0.05).The auditory thresholds of 0.25,0.5,1,2,4 kHz aided were declined 16.25±15.24,30.42±14.05,40.00±15.37,25.42±16.58,20.84±15.64 dB respectively.The SRT of three direction(0°,90°,270°)were 67.30±4.46,68.66±5.08,69.16±4.53 dB nHL versus aided for the day 39.40±4.97,37.40±5.27,41.76±8.19 dB nHL,aided for 2 weeks 37.51±5.06,37.24±7.09,40.42±7.46 dB nHL,aided for 3 months 38.32±4.92,38.72±6.34,42.65±5.68 dB nHL;3 For the above 6-year-old with unilateral profound sensorineural hearing loss group,the bone auditory thresholds PTA of injured ear can not calculate,but the bone auditory threshold PTA of normal ear was 9.37±6.80 dB HL.The soundfield PTA even with normal ear masked was 55.00±12.08 d B HL versus25.63 ±4.62,28.13 ±7.74,30.63±6.81 d B HL aided for the day,2 weeks and 3months,gained 29.38±14.31,26.88±11.61,24.38±11.84 dB respectively.The SRT of three direction(0°,90°,270°)were 54.15±2.96,55.47±3.32,54.60±3.04 dB nHL versus aided for the day 38.02±2.11,31.08±2.65,38.54±2.81 dB nHL,aided for 2 weeks 34.76±3.56,31.14±2.41,36.68±5.70 dB nHL,aided for 3 months 34.95±2.41,30.75±2.33,39.35±2.77 dB nHL;4 None of the patients had adverse reaction and adverse events during the using Sophono softband.Conclusions:1 The auditory hearing threshold and auditory development of patients who under 6-year-old with bilateral hearing loss have great improvement after using the Sophono softband.It can facilitate the language improvement significantly when using Sophono softband as early as possible.2 The audition and speech recognition threshold can be increased efficiently among the patients with bilateral conductive hearing loss 6 years old or above.So it will be a new choice for such patients.3 In the silence environment,using the Sophono softband not only provides sufficient hearing amplification but also has a good sense of direction among the patients with unilateral profound sensorineural hearing loss.
