Objective: To investigate the prognostic factors of liver metastasis from colorectal cancer,and to explore the survival time of patients with liver metastasis from colorectal cancer after treatment.Methods: This study retrospectively analyzed the clinicopathological features and follow-up time of 83 cases with liver metastasis from colorectal carcinoma who were inpatiently treated in Fourth Hospital of Hebei Medical University from January 2008 to January 2012.The Kaplan-Meier method was used for bivariate analysis,and the Cox regression model for multivariate analysis.Results:1 A total of 83 patients with liver metastases from colorectal cancer were included in the study,the 1,3,and 5 year survival rates of which were:57.8%,19.5%,7.6%,relatively,and the median survival time was 17.1months.2 Age,sex,primary site,T staging,regional lymph node metastasis,tumor differentiation degree,maximum diameter of liver metastases,number of liver metastases,distribution of liver metastases,hepatic metastasis time,CEA level,therapeutic method and blood vessel invasion were analyzed in this study.Kaplan Meier method was used for the single factor analysis.The analysis results showed that there were 6 factors showing an impact on patient survival: tumor differentiation degree,regional lymph node metastasis,CEA level,number of liver metastases,distribution of liver metastases,therapeutic method.The difference was statistically significant(P<0.05).The specific data were as follows:the 1,3 and 5 year survival rates in patients with high tumor differentiation was 57.9%,22.1%,9.2%,relatively,and the median survival time was 18.13 months;the 1,3 and 5 year survival rates of patients with low tumor differentiation was 57.1%,7.1%,0%,relatively,and themedian survival time was 13.77 months.The difference was statistically significant(P<0.05);The 1,3 and 5 year survival rates in patients with positive lymph node metastasis was 50.9%,14.5%,0%,relatively,and the median survival time was 12.73 months;the 1,3and 5 year survival rates of patients with negative lymph node metastasis is 71.4%,30.1%,21.5%,relatively,and the median survival time was 21.23 months,the difference was statistically significant(P<0.05).The 1,3 and 5 year survival rates in patients with CEA levels> 200ng/ml was 57.1%,7.6%,0%,and the median survival time was 13.77 months;The 1,3and 5 year survival rates of patients with CEA levels> 200ng/ml was 58.2%,25.2%,11.3%,relatively,and the median survival time was 19.7 months,the difference was statistically significant(P<0.05).The 1,3 and 5 year survival rates in patients with solitary liver metastasis was 68%,25.8%,12%,relatively,and the median survival time was 21.07 months;the 1,3and 5 year survival rates of patients with Multiple liver metastases was 42.4%,9.8%,0%,relatively,and the median survival time was 11.23 months.The difference was statistically significant(P<0.05).The 1,3 and 5 year survival rates in patients with simple leaf hepatic metastases was 63.9%,24.8%,12.0%,relatively,and the median survival time was 21.23 months;The 1,3and 5 year survival rates of patients who liver metastases were found in 2 hepatic lobes was 48.5%,12.1%,0%,relatively,and the median survival time was 11.36 months.The difference was statistically significant(P<0.05).The 1,3 and 5 year survival rates in patients who be treated surgically was 76.9%,50%,22.4%,ralatively,and the median survival time was 26.6 months;The 1,3and 5 year survival rates of patients with no surgical intervention was 52.7%,3.8%,0%,relatively,and the median survival time was 13.10 months.The difference was statistically significant(P<0.05).There are 6 factors not affecting the prognosis of patients: age,sex,primary site,blood vessel invasion,T staging,maximum diameter of liver metastases,and the difference was not statistically significant(P>0.05).3 These factors associated with prognosis were analyzed by the Cox regression model,and the analysis results showed that Lymph node metastasispositive,Poor tumor differentiation,Liver metastases in 2 hepatic lobes,No surgical intervention,were the independent risk factors of prognosis,while The CEA levels> 200ng/ml,and Multiple liver metastases were not the independent risk factors of prognosis.Conclusion:1 A total of 83 patients with liver metastases from colorectal cancer were included in the study.The 1,3,and 5 year survival rates were:57.8%,19.5%,7.6%,relatively,and the median survival time was 17.1 months.2 Single factor analysis showed that Poor tumor differentiation,Regional lymph node metastasis,The CEA levels>200ng/ml,Multiple liver metastases,Liver metastases in 2 hepatic lobes and No surgical intervention might mean a worse prognosis.3 Multiple factors analysis showed that Lymph node metastasis positive,Poor tumor differentiation,Liver metastases in 2 hepatic lobes and No surgical intervention,were independent risk factors of prognosis of liver metastasis from colorectal cancer.
